研究目的：

1.主要目的：探索课题一制定老年吞咽障碍团队协作模式的有效性和可行性，对吞咽障碍患者生存质量的影响。 2.次要目的：探讨老年人脑卒中疾病危险因素及功能障碍相关危险因素的团队化管理对老年脑卒中预后及患者家庭及社会医疗经济成本的影响。  

Objectives of Study：

1. Main purpose: to explore the effectiveness and feasibility of developing a teamwork model for the elderly with dysphagia, and its impact on the quality of life of patients with dysphagia. 2. Secondary objective: to explore the effect of team management of stroke disease risk factors and dysfunction-related risk factors in the elderly on the prognosis of elderly stroke and the family and social medical and economic costs of the elderly.

纳入标准：

Inclusion criteria

排除标准：

1.本次卒中前即存在吞咽功能障碍；
2.正在参与其他干预性研究；
3.无法配合康复治疗和评估；
4.合并其他严重脏器或系统性疾病；
5.合并严重精神疾病；
6.年龄大于90岁。

Exclusion criteria：

1. Dysphagia existed before the stroke;
2. Participating in other interventional research;
3. Unable to cooperate with rehabilitation treatment and evaluation;
4. Combined with other serious organ or systemic diseases;
5. Combined with severe mental illness;
6. Aged over 90 years.
