研究目的：

构建5种心脑血管疾病复发加重的中医风险预警模型，制定中医干预技术标准，应用于东部中心城市、中部县域、西部省会城市3000余例5种心脑血管疾病患者，管理1年，评判干预效果。  

Objectives of Study：

Established the risk of early-warning model and the criteria ofr intervention of the 5 kinds of cardiovascular and cerebrovascular diseases which were based on the system of the TCM. And were used to the 3000 patients of estern central city, central county and western provincial city of China,managing for 1 year and evaluted the effect of intervention.

纳入标准：

Inclusion criteria

排除标准：

1.有严重精神、心理疾病者；
2.认知功能障碍，沟通交流障碍或不配合者；
3.正在参与其他管理项目者。

Exclusion criteria：

1. Patients with severe mental illness;
2. Patients with severe cognitive impairment,serious communication deficiencies,or unable to match the researchers to complete the study;
3. Participated in other drug clinical trials or other study of chronic disease management now.
