研究目的：

本研究旨在评价护士为主导的以膳食摄入和含磷药物使用为目标（Targeting Intake & Pills，TIPs）的HD高磷血症患者多维血磷管理方案的初步成效。（1）TIPs方案能否改善HD高磷血症患者的血磷水平；（2）TIPs方案能否维持或改善HD高磷血液患者的营养状况。

Objectives of Study：

Objectives of Study To examine the preliminary effects of a nurse-led multi-component phosphate control (Targeting Intake & Pills, TIPs) intervention for hemodialysis patients. Its specific objectives are as follows. Primary objective To examine whether the nurse-led TIPs intervention results in lower serum phosphate levels than controlled care. Secondary objectives 1. To examine whether the nurse-led TIPs intervention results in lower dietary phosphorus intake than controlled care. 2. To examine whether the nurse-led TIPs intervention results in higher dietary protein intake than controlled care. 3. To examine whether the nurse-led TIPs intervention results in better medication adherence than controlled care. 4. To examine whether the nurse-led TIPs intervention results in lower intensity of pruritus than controlled care.

纳入标准：

Inclusion criteria

排除标准：

有明确的精神、心理疾病诊断；
合并严重并发症，如心力衰竭、严重感染、恶性肿瘤等其他疾病；
正参与其他临床研究。

Exclusion criteria：

1. Patients with definite diagnosis of mental and psychological diseases;
2. Patients with serious complications, such as heart failure, serious infection, malignant tumor and other diseases;
3. Patients who are participating in other clinical studies.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：	本研究采用病历记录表采集数据。研究参与者的所有资料为个人隐私，其全名不会出现在所有记录及文件中（只以姓名首字母及入选编号代表）。所有研究参与者的个人信息均属保密。这些信息可能包括参与者的姓名及其提供的其他个人信息。所有资料将会保密，一切资料的编码只有研究人员得悉。与本课题有关的资料将保存在有锁的档案柜中，电子信息会加密码保存，仅供研究人员查阅。为确保研究按照规定进行，必要时，政府管理部门或伦理审查委员会的成员按规定可在研究单位查阅研究参与者的个人资料。本研究结果发表时，将不会披露研究参与者的任何个人资料。
