研究目的：

环肺静脉消融术（circumference pulmonary vein isolation，CPVI）是房颤射频消融治疗的标准术式，但长程持续性房颤（long-standing persistent atrial fibrillation，LPAF）患者左房扩大、单纯CPVI治疗后房颤复发率高，CPVI联合左房后壁电隔离术（circumference pulmonary vein isolation associated with left atrial posterior wall electric isolation，CPVI-LAPWI）基于CPVI发展而来。本中心通过探讨对LPAF的患者行CPVI-LAPWI治疗的有效性，指导未来LPAF患者治疗指南方向。

Objectives of Study：

Using circumference pulmonary vein isolation associated with left atrial posterior wall electric isolation technique to evaluate outcomes in patients with long-standing persistent atrial fibrillation

药物成份或治疗方案详述：

术前服用抗凝药（华法林控制INR 2-3之间；达比加群 110mg bid；利伐沙班 15mg qd）治疗，完善经胸超声明确左房大小，完善经食道超声明确左心房有无血栓及有无严重心脏结构异常。术中利多卡因局麻，穿刺双侧股静脉，左腿放置6F、7F短鞘（Bard, Lowell, MA），X线下放置冠状窦电极；右腿放置长鞘（SL1, St. Jude），使用房间隔穿刺针刺房间隔，放置大头及pentaray至左心房。在Carto系统（CARTO, Biosense Webster）下行左房建壳，在35W、43度、盐水模式（盐水2ml/min、消融状态下17ml/min）下行CPVI，其中消融左房前壁时AI（Ablation index，射频消融指数） 500、左房后壁时AI 400，后行左房后壁顶部线及底部线线性隔离，其中消融顶部线时AI 450、下壁线时AI 400。消融后若房颤持续，予患者力月西2mg后电复律，并于窦律下用pentaray行精细高密度标测确认消融线完全阻滞并标测后壁内最早激动点。若后壁内电激动标测没有明确早的部位，选择高电压区域大电位开始消融（图5 CPVI-LAPWI后选择高电压区域大电位开始消融），再次用pentaray精细高密度标测确认后壁隔离。术中予患者低分子肝素抗凝，维持ACT 300-350。术后口服奥美拉唑1月，胺碘酮3月，抗凝药6月。

纳入标准：

Inclusion criteria

排除标准：

(1)左心耳血栓；
(2)心脏结构严重异常（中、重度二尖瓣、三尖瓣、主动脉瓣狭窄、返流及畸形；房间隔、室间隔缺损修补术后；法洛四联症；风湿性心脏病等）；
(3)无法配合随访；
(4)肌酐清除率<30ml/min；
(5)感染性休克；
(6)恶性肿瘤晚期；
(7)孕妇；
(8)心包压塞或大量心包积液；
(9)心脏射频消融治疗及外科手术史的患者；
(10)左房前后径>50mm；
(11)对比剂过敏。

Exclusion criteria：

1. left atrial appendage thrombosis;
2. abnormal cardiac structures disease (i.e. severe mitral, tricuspid, aortic malformation; atrial, ventricular septal defect; tetralogy of Fallot);
3. heart failure;
4. history of RFCAcryoballon ablation and cardiac surgical procedures;
5. mental disease;
6. eGFR<30ml/min;
7. septic shock;
8. advanced malignant tumor;
9. pregnancy;
10. cardiac tamponade or major hydropericardium;
11. left atrial anteroposterior diameter larger than 50 mm.
