研究目的:

针对冠心病抗栓治疗中存在的血栓事件和出血风险,建立集成日常和临床数据采集、传输、处理、分析以及预警、干预等全程健康管理功能的大数据系统,以解决科学评价抗栓治疗的疗效与安全性问题,建立评价和预测血栓/出血风险的方法,并指导个体化抗栓治疗,探索合并糖尿病、肾功能不全等高危病人的优化抗栓策略。  

Objectives of Study:

In view of the thrombotic events and bleeding risks in the anti thrombotic treatment of coronary heart disease, a big data system integrating daily and clinical data collection, transmission, processing, analysis, early warning, intervention and other whole process health management functions is established to solve the problems of scientific evaluation of the efficacy and safety of anti thrombotic treatment, establish methods to evaluate and predict the risk of thrombus / bleeding, and guide individual anti thrombotic treatment To explore the optimal strategy of antithrombotic therapy for high-risk patients with diabetes mellitus and renal insufficiency.

药物成份或治疗方案详述:

冠心病患者的血栓及出血事件是一个逐渐发展的过程,这个过程和众多因素成关联或者因果关系,比如饮食习惯、生活行为,用药的种类和剂量、治疗的依从性等,从病理生理学的角度观察,这种变化是从人体组织器官的功能性变化逐渐演变到器质性病变,对应的人体生物物理信息必然也发生变化。前苏联宇航局为了监测宇航员在太空中的生理参数,使用生物电阻抗测量技术监控人体在太空中人体组织器官的功能是否发生变化,针对冠心病抗栓的具体应用,本项目进一步使用无创脉搏波波形分析技术检测动脉僵硬度,用以监测具体器官的早期器质性病变,在出血和血栓事件发生后利用专业医疗机构的临床数据,包括出血及血栓相关的各项化验资料和特殊检查资料,建立日常行为到医疗事件的全程闭环监测体系,并获得完整数据闭环. 本项目通过可穿戴设备采集总共 20000 例样本冠心病患者的日常数据,并抽取专业医疗机构数据库中的对应患者的临床数据,把不同来源、格式、特点性质的数据在逻辑上和物理上有机地集中。经过数据清理,将完整、正确、一致的数据信息存入数据仓库中。数据仓库是面向主题的、集成的、与时间相关且不可修改的数据集合,本项目根据研究目的建立两个主题数据仓库,分别是冠心病出血及血栓数据仓库和抗栓疗效数据仓库。 

Description for medicine or protocol of treatment in detail:

Thrombus and haemorrhage in patients with coronary heart disease is a process of gradual development, which is related or causal with many factors, such as diet habits, life behaviors, types and doses of drugs, treatment compliance, etc. from the perspective of pathophysiology, this change is gradually evolving from functional changes of human tissues and organs to organic diseases, corresponding to The biophysical information of human body must also change. In order to monitor the physiological parameters of astronauts in space, the former Soviet space agency used bioelectrical impedance measurement technology to monitor whether the function of human tissues and organs changed in space. For the specific application of antithrombotic therapy for coronary heart disease, the project further used noninvasive pulse wave analysis technology to detect arterial stiffness to monitor the early organic lesions of specific organs. After the occurrence of bleeding and thrombus events, a closed-loop monitoring system from daily behavior to medical events was established by using the clinical data of professional medical institutions, including the laboratory data and special examination data related to bleeding and thrombus, and a complete closed-loop data was obtained In this project, the daily data of 20000 patients with coronary heart disease were collected by wearable devices, and the clinical data of corresponding patients in the database of professional medical institutions were extracted. The data of different sources, formats and characteristics were logically and physically integrated. After data cleaning, the complete, correct and consistent data information is stored in the data warehouse. Data warehouse is a subject oriented, integrated, time-dependent and non modifiable data set. According to the research purpose, this project establishes two subject data warehouse, which are coronary heart disease bleeding and thrombus data warehouse and antithrombotic efficacy data warehouse. 

纳入标准:

Inclusion criteria

排除标准:

有全身性活动性感染的患者;严重肝肾功能不全(ALT>135mmol/L,Cr>200umol/L)或慢性阻塞性肺病患者;有明显出血倾向及血液性疾病的患者;恶性肿瘤及终末期疾病的患者;病人预期寿命小于 6 个月;合并心脏瓣膜或左室室壁瘤等其它疾病需同期手术者;有脑血管疾病者(目前或者六个月内有过脑梗死、脑出血);最近参加试验药物研究而未完成主要终点或临床上妨碍临床研究终点者。

Exclusion criteria:

Patients with systemic active infection; patients with severe hepatorenal insufficiency (ALT > 135mmol / L, Cr > 200umol / L) or chronic obstructive pulmonary disease; patients with obvious bleeding tendency and hematologic diseases; patients with malignant tumors and end-stage diseases; patients with life expectancy less than 6 months; patients with heart valve or left ventricular aneurysm and other diseases requiring simultaneous operation; patients with cerebrovascular diseases (at present or within six months, there has been cerebral infarction or cerebral hemorrhage); recently participated in the trial drug research but failed to complete the main end point or clinically hindered the clinical research end point.
