研究目的：

本研究拟在胸外科肺癌患者中针对4L组淋巴结清扫开展前瞻性临床研究，评估清扫4L组淋巴结的可操作性以及围术期安全性，了解清扫4L组淋巴结的必要性和重要性，并明确清扫4L组淋巴结对患者短期、中期和远期预后的影响。  

Objectives of Study：

The purpose of this study is to carry out a prospective clinical study on 4L group lymph node dissection in patients with lung cancer in thoracic surgery, evaluate the operability and perioperative safety of 4L group lymph node dissection, understand the necessity and importance of 4L group lymph node dissection, and clarify the impact of 4L group lymph node dissection on the short-term, medium-term and long-term prognosis of patients.

纳入标准：

Inclusion criteria

排除标准：

（1）术前穿刺或术中冰冻证实病灶为小细胞肺癌或转移性肿瘤；
（2）同侧肺叶或者不同肺叶多原发病灶；
（3）两站或多站淋巴结转移、N3淋巴结转移或者远处转移；
（4）术前检查发现心肺功能不能耐受手术或有严重基础疾病，如严重高血压，糖尿病，神经及精神系统疾病等而无法进行手术者；
（5）术中病灶不能行完整切除、或姑息性切除者；
（6）失访或自愿退出本研究者；
（7）一般状态较差，生活无法自理，无法接受手术者。

Exclusion criteria：

(1) small cell lung cancer or metastasis is confirmed by preoperative puncture or intraoperative freezing;
(2) multiple primary lesions in the same or different lobes;
(3) two or more station lymph node metastasis, N3 lymph node metastasis or distant metastasis;
(4) the preoperative examination found that the cardiopulmonary function could not tolerate the operation or there were serious basic diseases, such as serious hypertension, diabetes, neurological and mental system diseases, which could not be operated;
(5) the lesions could not be excised completely or palliatively;
(6) those who lost the interview or voluntarily quit the study;
(7) those who are generally in poor condition, unable to take care of their own life, and unable to accept surgery.
