研究目的：

评估完全经肛门直肠全系膜切除术治疗直肠癌的可行性和安全性,以及其在取得腹部无疤痕的最大的美容效果的同时，是否具有疼痛轻、术后恢复快、术后排尿排便、性功能受损小，以及生活质量受影响小等优点。

Objectives of Study：

The investigators conduct this study, looking to see if this purely transanal NOTES investigational procedure is a safe and effective approach to radically remove rectal cancer of the mid and lower rectum and see if it can reduce pain, gain faster recovery and better function and life quality when gaining the most improved cosmesis.

纳入标准：

Inclusion criteria

排除标准：

1、合并肠梗阻、肠穿孔、肠出血等需要急诊手术切除的病人；2、肿瘤侵犯外括约肌及肛提肌或累及邻近器官需要联合脏器切除者；3、怀孕或哺乳期妇女；4、术前肛门功能差、大便失禁者；5、严重的精神疾病；6、具有炎症性肠病病史；7、近期诊断合并其他恶性肿瘤；8、严重的肝肾功能、凝血功能障碍，合并严重的基础疾病等不能耐受手术者；9、术前有感染未控制者；10、患者或家属无法理解本研究的条件和目标

Exclusion criteria：

1. obstructing, perforated or hemorrhagic rectal cancer require for emergent surgery;
2. T4 tumor that invade the external sphincter or levator ani muscle or neighbor organs that should undergo multivisceral resection;
3. pregnant or breast-feeding women;
4. fecal incontinence;
5. mental disorder or disease;
6. history of inflammatory bowel disease;
7. other malignancies diagnosed within the previous year;
8. severe impaired renal and hepatic function, coagulopathy or other severe comorbidities that cannot tolerate surgery;
9. any evidence of active infection;
10. cannot reach written informed consent.
