Objectives of Study：

Coordinate experts in multidiscipline to establish a protocol of hospital outreach intervention for COPD patients; 1. Provide hospital outreach intervention for patients with COPD after discharge; 2. Evaluate the effectiveness in preventing re-hospitalization and decreasing medical cost; 3. Evaluate the cost-effectiveness of the hospital outreach intervention.

纳入标准：

Inclusion criteria

排除标准：

患有其他严重干扰本病发展的疾病；
受文化水平或其他因素制约，不能正确回答相关问题、提供资料、接收信息；
稳定期尚且不能行走。

Exclusion criteria：

Patents with characteristics that can confound the analysis of the primary outcome;
Patients with an inability to provide good data or follow commands;
Patients with an inability to walk when their COPD is stable.
