研究目的:

坏死性小肠结肠炎(Necrotizing enterocolitis,NEC)是早产儿常见的胃肠道危重症,病死率和后遗症发生率高,早期诊断、正确评估是防治的关键和难点。本项目将通过同质化的多中心研究,采用流行病学调查、前瞻性巢式病例对照研究等方法,对疾病的危险因素、临床表现、实验室指标、影像学检查、生物学标记物、肠道菌群多态性等进行综合评价,探索动态监测肠道血流动力学指标、粪钙卫蛋白、人β防御素-2、肠道菌群多态性等新技术新指标在疾病早期诊断中的价值,构建动态贝叶斯网络模型,生成疾病早期预测模型,通过临床验证、优化、推广应用,提高早产儿的存活率和生存质量。  

Objectives of Study:

Necrotizing enterocolitis (NEC) is an inflammatory bowel disease of neonates withsignificant morbidity and mortality in preterm infants and remains one of the most common gastrointestinal emergencies in newborn infants. Due to the multifactorial nature of the disease, early diagnosis and accurate assessment remain challenging. We are going to apply a multicenter, epidemiological investigations and nested case-control studies to assess the values of various factors including baseline characteristics, clinical manifestations, laboratory indicators, imaging examinations, biological markers in early diagnosis and evaluation of NEC in preterm infants. This project will explore the value of dynamic monitoring of intestinal hemodynamics, serial measurement of fecal calprotectin, human beta-defensin-2 and intestinal flora polymorphism in early diagnosis and evaluation of NEC. In order to deal with the complexity and uncertainty in diagnosiswe will also apply dynamic Bayesian network to build a prediction model to predict the disease in preterm infants. After clinical verification and optimization, the established models will provide new ideas for the prevention and treatment of NEC to improve the survival rate and long-term prognosis of premature infants.
