研究目的：

（1）构建一个基于夫妻糖尿病协同管理模型（CCMM），以满足中国老年患者及其社区管理者的需求; （2）评估CCMM在促进夫妻健康中的作用； （3）基于RE-AIM，探索该试验实施科学相关情况。  

Objectives of Study：

We aim to (1) construct a Couple-based Collaborative Management Model （CCMM） based on theories and tailored to the needs of Chinese older patients with T2DM and their partners living in communities; (2) evaluate the effects of CCMM in promoting health and wellbeing of the couples; (3) explore implementation-related outcomes.

纳入标准：

Inclusion criteria

排除标准：

(1) 以前参加过T2DM教育课程的患者将被排除在外；
(2) 为了进一步区分患者和配偶，我们进一步排除了患有糖尿病的夫妇。

Exclusion criteria：

1. Patients who previously participated in T2DM education courses;
2. We further exclude couples that both have diabetes, for a clear distinction between patients and spouses.

不同社区健康服务中心的每一位参与者将会被以1:1的比例随机分配至夫妻组和个人组。统计分析人员将会按患者性别和年龄组进行分层随机化，同时进行层序随机化。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Participants of each community health center will be randomized 1:1 within center into a couple-based arm and an individual-based arm. Stratified randomization by patients’gender and age group will be performed alongside randomization of the class sequence,by a statistician otherwise not involved.

Blinding：

It is an open-label trial.Outcome assessors will be blinded to the group assignments and will be different from care managers who conduct and monitor the interventions. Data analysts will remain blinded to the group assignment throughout the study.
