1. 为了明确tDCS和认知训练在卒中人群中的干预效果; 2. 为了明确TMS在卒中人群中的干预效果;  

Objectives of Study:

1. One aim of this study is to know the effect of tDCS and cognitive training in stroke patients. 2. Another aim of this study is to know the effect of TMS in stroke patients.

纳入标准:

Inclusion criteria

排除标准:

1.卒中前精神或神经科疾病,包括焦虑症、注意缺陷多动障碍、帕金森病; 2.有严重的抑郁症病史(在卒中之前); 3.有癫痫病史(在卒中之前); 4.有癫痫家族史; 5.在卒中前3个月有晕厥病史; 6.会影响完成认知任务的严重的听力丧失、视觉及运动障碍; 7.已知正在怀孕者; 8.服用影响皮质兴奋性的药物或消遣性毒品; 9.颅内或颅内有金属(除了钛)或者电极植入(金属片、夹子、耳蜗植入、深部脑电极和药物泵); 10.其他部位有金属(除了钛)或者电极植入(如心脏起搏器、外伤性医学残留金属片); 11.刺激部位的皮肤问题,如皮炎、湿疹和银屑病; 12.过去6个月有经颅刺激; 13.6个月前有行经颅电刺激或经颅磁刺激不良反应; 14.颅骨骨折、明显的颅骨缺损以及刺激部位下的大血管闭塞; 15.卒中发病时有癫痫或者实验前有癫痫; 16.影响认知训练的严重的认知障碍。

Exclusion criteria:

1. Pre-stroke psychiatric or neurological disease by self-report (e.g., anxiety disorder, ADHD, Parkinson's disease, etc.);
2. History of diagnosed severe depression (diagnosed pre-stroke);
3. History of epilepsy (diagnosed pre-stoke);
4. Family history of epilepsy;
5. Fainting spells or syncope in the last three years pre-stroke;
6. Significant hearing loss, vision or motor impairment that would prevent them from performing the task;
7. Known to be pregnant;
8. Consuming medication affecting cortical excitability or recreational drugs;
9. Metal (except titanium) or electronic implants in the brain /skull (e.g., splinters, fragments, clips, cochlear implant, deep brain stimulation, medication pump);
10. Metal (except titanium) or any electronic device at other sites in your body, such as cardiac pacemaker or traumatic metallic residual fragments;
11. Skin problems such as dermatitis, psoriasis or eczema under the stimulation sites;
12. Brain stimulation in the past six months;
13. Undergone transcranial electric or magnetic stimulation in the past (more than 6 months) which resulted in adverse effects;
14. Skull fractures, significant skull defects, skull plates or large vessels occlusions in the site of electrode placement;
15. Seizure at the time of stroke or between the stroke and starting of the therapy;
16. Severe cognitive problem which affect the understanding of training task.

Blinding:

Trial participants were blinded to group assignment. Sham tDCS stimulation included a brief ramp-up and ramp-down procedure to mimic the sensation of active current. For sham TMS stimulation, the edge of the coil was kept perpendicular to the head.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

数据来源于两个主要渠道:(1)病例报告表(CRFs),用于记录临床及人口学研究信息;(2)基于计算机的任务记录,用于采集行为表现数据,包括正确率和反应时(RT)。数据采集完成后,将其整合至研究数据库中,经过错误检查和缺失值处理、数据清洗等步骤后用于后续分析,并以去标识化的形式进行存储和共享。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Data were collected from two main sources: (1) Case Report Forms (CRFs) for clinical/demographic study information, and (2) computer-based task recordings performance data, including accuracy and reaction time (RT). After collection, the data were integrated into a study database, checked for errors and missing values, cleaned for analysis, and maintained in de-identified form for storage and sharing.
