研究目的：

本研究拟在分娩性臂丛神经损伤合并肩内旋畸形和主动外展受限的患儿中，通过随机对照研究探讨肉毒毒素注射大圆肌、肩胛下肌联合物理治疗的治疗方式是否能有效改善肩内旋畸形和肩关节主动外展，其疗效是否优于单纯物理治疗，这是本研究拟解决的关键科学问题。  

Objectives of Study：

To investigate the effectiveness of botulinum toxin A injection of teres major and subscapularis in addition to physical therapy for internal rotation contracture in children with obsteric brachial plexus palsy, and to present the best evidence through a randomized controlled study.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

Inclusion criteria

排除标准：

1、被研究的肢体曾使用肉毒毒素注射，或进行过骨科矫形手术（如背阔肌、大圆肌移位术，或肩胛下肌切断术）；
2、任意一侧上肢有外伤、骨折或神经损伤；
3、合并存在中枢神经系统疾病或损伤史；
4、注射部位存在感染或皮肤病变；
5、任何可能使受试者在使用肉毒毒素治疗时风险升高的医疗情况，包括诊断为重症肌无力，Eaton-Lambert综合征，肌萎缩性脊髓侧索硬化，或其他可能干扰神经肌肉功能的疾病；
6、合并使用氨基糖苷类抗生素或其他可能干扰神经肌肉功能的药物；
7、根据研究者的判断，受试者的合并疾病如不稳定的心脏、肺脏、肝脏或肾脏疾病会使其面临重大风险，可能影响研究结果，或者可能严重干扰研究进行；
8、正在参与或在入组本研究前30天内参加了其他临床研究。

Exclusion criteria：

1. previous botulinum toxin injection or orthopedic surgery (e.g. muscle transfer or cutoff) on the affected arm,
2. previous trauma,fracture or nerve injury on either side of the arm;
3. concurrent central nervous system disease or injury;
4. infection or skin disease on the injection site;
5. in any situation which increase the risk of botulinum toxin injection,including myasthenia gravis, Eaton-Lambert syndrome,or other diseases interferring the nerve-muscle function;
6. concurrent use of aminoglycoside antibiotics or other medications interferring the nerve-muscle function;
7. concurrent severe cardiac,pulmonary, hepatic or renal diseases which may increase the risk,or affect the study result or disturb the study;
8. involved in other clincial study or has took part in other study in 30 days before enrollment of this study.

随机方法（请说明由何人用什么方法产生随机序列）：

通过计算机产生1-94随机序列数1组，按入组病人的就诊顺序取随机序列数，能被2整除者入对照组，不能被2整除者入试验组(1:1分组）。各中心筛选受试者后通知陈婵医生，由陈婵医生负责随机分组，并在研究开始前通知到各中心负责人。

1-94 random sequence array was generated by a computer-based software. The random sequence numbers were taken according to the order of subject enrollment and assigned 1：1 to the 2 group before the intervention.

因幼儿研究特殊因素，幼儿父母（监护人）一般不愿意进行安慰剂对照双盲研究。因此本研究只对访视评价者和参与物理/作业治疗人员采取盲法。
