本研究的目的是评估糖尿病自我管理支持干预项目对糖尿病患者血糖控制的效果。  

Objectives of Study：

The aim of this study is to test the effects of diabetes self-management support intervention for patients with type 2 diabetes in glycemic control.

纳入标准：

Inclusion criteria

排除标准：

（1）合并严重心、脑、肾、眼、足和神经系统并发症者；
（2）行动不便、神志不清、精神异常的患者；
（3）其他严重疾病的患者（例如晚期癌症）;
（4）正在参加其他类似研究课题的患者。

Exclusion criteria：

(1) Had serious complications of diabetes;
(2) Had dementia or mental disorder, and cannot cooperate with researchers;
(3) Had other serious disease, such as advanced cancer.
(4) were Participating in other similar study.

采用区组随机的方式进行分组。采用随机数表法，将来自同一个社区中心的3个卫生服务站被随机分配至任何一个干预组中。该过程由Excel软件完成。

Block randomization was used, 3 community healthcare service stations from the same center were Randomly assigned to one of the 3 intervention groups using random digits table. The process was completed by Excel software.
