研究目的：

1.观察综合吞咽训练对脑卒中合并OSA及吞咽障碍患者的口咽部形态学变化、AHI等PSG指标及吞咽障碍评估指标的影响，从而为脑卒中合并OSA患者的OSA治疗提供一种新的途径。 2.观察脑卒中合并OSA及吞咽障碍患者的PSG指标、口咽部形态学及吞咽障碍评估指标的相关性，为卒中后合并OSA患者的OSA筛查提供了一个新思路。  

Objectives of Study：

1. To observe the effects of comprehensive swallowing training on oropharyngeal morphological changes, AHI and other PSG indexes and evaluation indexes of swallowing disorders in patients with stroke complicated with OSA and dysphagia, so as to provide a new approach for OSA treatment in patients with stroke complicated with OSA. 2. To observe the correlation of PSG index, oropharyngeal morphology and evaluation index of swallowing disorder in patients with stroke complicated with OSA and dysphagia, and provide a new idea for OSA screening in patients with stroke complicated with OSA.

药物成份或治疗方案详述：

吞咽护理：口腔冰刺激，口唇运动，自由饮水计划，监测和指导患者每日进食，间歇性经口插胃管进食（若需要）。 综合吞咽干预：吞咽运动训练，神经肌肉电刺激治疗，发声训练。 

Description for medicine or protocol of treatment in detail：

Swallowing care: Oral ice stimulation, Lip movement, Free water plan, Monitor and guide daily eating, Intermittent transoral gastric tube feeding（if disired）. Comprehensive swallowing intervention: Swallowing movement training, Neuromuscular electrical stimulation, Vocal training.  

纳入标准：

Inclusion criteria

排除标准：

1 重度卒中者（NIHSS≥16分）；
2 重度言语障碍；
3 重度认知障碍（MMSE<9分）；
4 中枢性睡眠呼吸暂停；
5 既往夜间呼吸暂停或未觉醒；
6 既往白天严重嗜睡（Epworth嗜睡程度评分>16分）；
7 既往颌面部畸形、咽腔狭窄、鼻部异常；
8 既往曾有CPAP治疗；
9 有严重心肺功能疾患或急性疾病（失代偿性心衰，3个月内曾有心脏或呼吸停止或心梗，严重肺炎，难治性高血压，气胸，大泡性肺气肿）。

Exclusion criteria：

1 patients with severe stroke (NIHSS>=16 points);
2 severe speech disorder;
3 severe cognitive impairment (MMSE<9)
4 central sleep apnea;
5 previous night apnea or micro-awakening;
6 previous severe daytime sleepiness (Epworth Sleepiness Score >16);
7 previous maxillofacial deformity, pharyngeal cavity stenosis and nasal abnormalities;
8 having had CPAP treatment in the past.
9 severe cardiopulmonary function disorders or acute diseases (decompensated heart failure, had heart or respiratory arrest or myocardial infarction within 3 months, severe pneumonia, refractory hypertension, pneumothorax, alveolar emphysema).
