To verify the promotion of geriatric medical technology under the service environment of combining medical care with nursing and improve the health status of the frail elderly in the implementation area.

Inclusion criteria

排除标准：

1）不愿参加本研究；
2）将要离开研究地区居住；
3）预期寿命小于1年的老人。

Exclusion criteria：

1) unwilling to participate in this study;
2) to leave the study area to live;
3) old people with a life expectancy of less than one year.
