研究目的：

快通道麻醉是小儿先天性心脏病矫正畸形的优选方案，与传统手术比较，可于术后早期拔除气管导管，降低呼吸机相关并发症发生率，缩短住院时间，加快患者康复。 但由于心脏手术本身的特点，术中及术后需要大剂量的阿片类药物，抑制疼痛及心血管应激反应，这不可避免会延迟术后恢复，增加小儿先天性心脏病快通道麻醉管理难度。 超声引导下胸椎旁神经阻滞具有胸段硬膜外麻醉相似的镇痛效果，目前已经广泛运用于胸科手术麻醉及术后镇痛。因此，本研究拟评价超声引导下胸椎旁阻滞在小儿先天性心脏病外科手术超快通道麻醉中的效果，寻求小儿心脏外科手术超快通道麻醉的更优方法。  

Objectives of Study：

Fast-track anesthesia is the preferred method for correcting congenital heart diseases in children. Compared with traditional surgery, it can remove tracheal tubes early after surgery, reduce the incidence of ventilator-related complications, shorten hospitalization time and speed up the recovery of patients. However, due to the characteristics of cardiac surgery itself, large doses of opioid drugs are required during and after the surgery to inhibit pain and cardiovascular stress response, which inevitably delays postoperative recovery and increases the difficulty of fast channel anesthesia management for congenital heart disease in children. Ultrasound-guided thoracic paravertebral nerve block has similar analgesic effect to thoracic epidural anesthesia, and has been widely used in thoracic surgery anesthesia and postoperative analgesia. Therefore, this study intends to evaluate the effect of ultrasound-guided thoracic paravertebral block in ultra-fast channel anesthesia for pediatric congenital heart disease surgery, and to seek a better method of ultra-fast channel anesthesia for pediatric cardiac surgery.

纳入标准：

Inclusion criteria

排除标准：

1）ASA 4-5级，RACHS 5-6级； 2）术前因心衰肺炎上机治疗； 3）肝肾疾病，有出凝血障碍； 4）发育差的早产儿； 5）外科畸形矫治不满意； 需多种药物支持，血流动力学不稳定

Exclusion criteria：

1) Preoperative treatment for heart failure pneumonia;
2) Liver and kidney diseases, with clotting disorders;
3) Poor premature infants;
4) Unsatisfactory treatment of surgical deformities Multiple drug support, hemodynamic instability.
