研究目的：

1)研究艾滋病肺孢子菌肺炎并发急性呼吸衰竭的呼吸力学特征； 2)优化艾滋病肺孢子菌肺炎并发急性呼吸衰竭的有创通气方案。

Objectives of Study：

1) To study the respiratory mechanical characteristics of PCP complicated with ARDS in AIDS; 2) To optimize the invasive ventilation scheme of PCP complicated with ARDS in AIDS.

药物成份或治疗方案详述：

北京地坛医院2019年7月起收治的艾滋病PCP并发急性呼吸衰竭并需要有创机械通气病例，共收集42例。 1) 呼吸力学测定：首先按照ARDS保护性通气的原则给予初始通气，设置呼吸机参数潮气量为6ml/kg理想体重、PEEP为5cmH2O、FiO2为100%，同时进行镇静和循环支持，建立PICCO血流动力学和食道测压系统。在血流动力学稳定的情况下，给予肌肉松弛剂，应用慢流速法测定准静态压力-容积曲线（P-V曲线），进而得到患者的呼吸力学参数。 2) 呼吸支持方法：按照入院顺序分为两组，两组均采取小潮气量（6ml/kg）。 A组（量表法组）：依据ARDSnet提供的conversative PEEP/FiO2量表法（见表-1），来调节PEEP和FiO2。保证PaO2在60-80mmHg或者SpO2在90%～95%，此时所选择的PEEP即为最佳PEEP。入组后0小时、24小时、48小时、72小时，分别监测呼吸力学参数和血流动力学参数。表-1 低PEEP/高FIO2方案(ARDSnet) FiO2 0.3 0.4 0.5 0.6 0.7 0.8 0.9 1.0 PEEP 5-8 8 8-10 10 10-14 14 14-18 18-24 B组（跨肺压组）：根据食道压来计算跨肺压B组（跨肺压组）：根据食道压来计算跨肺压，跨肺压=气道平台压-胸腔内压（食道压）。每次PEEP上调2cmH2O，每10分钟调节1次，直至呼气末跨肺压稳定在0～5cmH2O之间。潮气量为6ml/kg，吸气末跨肺压小于25cmH2O。入组后0小时、24小时、48小时、72小时，分别监测呼吸力学参数和血流动力学参数，并采集血液送实验室检测IL-6、IL-10、TNF等细胞因子水平。观察如下指标： 1)主要指标：机械通气 72 小时的氧合指数（PaO2/FiO2）。 2)次要指标：预后指标：28天病死率、60天病死率；机械通气并发症：气胸、支气管胸膜瘘、VAP等。

纳入标准：

Inclusion criteria

排除标准：

1）既往有慢性肺疾病（COPD、哮喘、支气管扩张）；
2）心源性肺水肿（超声心动图或PICCO监测诊断）；
3）血流动力学不稳定（经扩容和升压药支持后SBP低于90mmHg和组织灌注不足者，或SBP>180mmHg）；
4）存在活动性出血；急性冠脉综合征一周内，或存在严重心律失常；
5）严重颅内高压或神经肌肉病变；
6）妊娠；
7）食道胃底病变或手术患者；
8）存在呼吸力学测定禁忌症；
9）存在气胸、皮下气肿和明显肺大泡。

Exclusion criteria：

1) past medical history with chronic pulmonary diseases (COPD, asthma, bronchiectasis);
2) cardiogenic pulmonary edema (diagnosised by echocardiography or PICCO monitoring);
3) hemodynamic instability (SBP is lower than 90mmHg and tissue perfusion is insufficient after volume expansion and pressure booster support, or SBP > 180mmHg);
4) active bleeding exists;Acute coronary syndrome within one week, or presence of severe arrhythmia;
5) severe intracranial hypertension or neuromuscular lesions;
6) pregnancy;
7) patients with esophageal and gastric fundus lesions or surgery;
8) contraindications to respiratory mechanics;
9) pneumothorax, subcutaneous emphysema and obvious pulmonary bullae were present.
