研究目的：

先心矫治手术快速康复策略目标是术后早期拔管同时减少围术期免疫应激反应，促进患者早期康复。围术期应用右美托咪啶充分镇静镇痛有利于早期拔管，减少术中应激反应免疫反应，减少心血管不良反应。体外循环心脏手术期间血液与心肺转流机异物表面接触、缺血再灌注损伤及内毒素的作用等都可引发全身炎症反应。静脉麻醉和七氟烷吸入等全身麻醉也均可引起术后炎症反应。小儿心脏手术术后心肺功能不全常与全麻围术期大量释放的炎性介质相关。因而减轻围术期炎症反应可提高小儿心脏手术术后恢复质量。右美托咪定应用于麻醉诱导期至术后48小时的围术期镇静镇痛对于围术期免疫反应及应激反应的影响有待进一步研究。

Objectives of Study：

The goal of the rapid recovery strategy of preoperative orthopaedic surgery is to reduce perioperative immune stress and promote early recovery. The application of right metomidine in the perioperative period is conducive to early tubing, reducing the stress response immune response and reducing cardiovascular adverse reactions. Extracorporeal circulatory heart surgery, blood and cardiopulmonary bypass foreign body contact, ischemia reperfusion injury and the role of endotoxin can trigger systemic inflammatory reactions. General anesthesia such as intravenous anesthesia and sevoflurane inhalation can also cause postoperative inflammatory reactions. Cardiopulmonary insufficiency after pediatric heart surgery is often associated with a large amount of inflammatory media released during general anesthesia perioperative period. Therefore, reducing the perioperative inflammatory response can improve the quality of postoperative recovery in children with cardiac surgery. The effects of dextrometadine on perioperative immune response and stress response during the perioperative period from anesthesia induction to 48 hours after operation need to be further studied.

纳入标准：

Inclusion criteria

1) Aged one month to three years, weight 5 kg to 20 kg;
2) ASA I-III;
3) Precocious heart disease surgical risk rating: RACHS level 1-4(RACHS level 3-4 should be cautious);
4) Surgical malformation treatment satisfaction(TEE evaluation anatomy and circulation);
5) Assessment of Breathing Cycle Function at Stop Time:
Blood gas: electrolytes; Lung ventilation index ventilation index(carbon dioxide partial pressure, oxygen partial pressure, oxygenation index);
P-V ring assessment of lung compliance(especially in preoperative pneumonia);
Direct pulmonary arterial pressure measurement(especially in children with preoperative pulmonary hypertension);
Circulatory status assessment(application of vasoactive drugs, systolic pressure, mean pressure, pulse pressure difference, EF value, myocardial surface color and myocardial contraction state, pacemaker adjustable arrhythmia, lactic acid value);
6) Extraction time assessment of respiratory circulation function:
Breathing(frequency, Tidal volume, oxygen saturation, lung ventilation index);
Circulation(application of vascular active drugs, systolic pressure, mean pressure, pulse pressure difference, TEE)
Blood gas(acid-base electrolytes, lactic acid);
pupil;
Urine volume and color.

排除标准：

1）ASA 4-5级，RACHS 5-6级；
2）术前因心衰肺炎上机治疗；
3）肝肾疾病，有出凝血障碍；
4）发育差的早产儿；
5）外科畸形矫治不满意；
需多种药物支持，血流动力学不稳定

Exclusion criteria：

1) ASA Level 4-5, RACHS Level 5-6
2) Preoperative treatment for heart failure pneumonia
3) Liver and kidney diseases, with clotting disorders;
4) Poor premature infants
5) Unsatisfactory treatment of surgical deformities
Multiple drug support, hemodynamic instability

由研究者使用SPSS软件生成5个随机数字表，根据随机数将接受心脏外科手术的患儿分配为5组：1.常规全身麻醉组2.椎旁阻滞组3.椎旁阻滞复合右美托咪啶组4.右美托咪啶组5.术后追加右美托咪啶组。将随机化结果在麻醉前告知主管麻醉医师。
