研究目的: |
肝移植术后认知功能障碍(postoperative cognitive dysfunction,POCD)等神经精神系统并发症影响患者生存率和生活质量。右美托咪定可能会降低POCD的发生,静息态功能磁共振成像(resting state functional magnetic resonance imaging, rs-fMRI)是研究脑结构与功能的最新影像学方法,关于右美托咪定对肝移植患者术后认知功能与静息态功能磁共振成像关系的研究尚未见报道。本课题拟通过观察右美托咪定对肝移植患者术后认知功能、脑损伤的血清标志物以及脑静息态空间网络连接和功能的影响,探讨右美托咪定对肝移植患者术后认知功能的影响及可能的中枢机制,为优化临床路径提供依据。 |

Objectives of Study: |
Neuropsychiatric complications such as postoperative cognitive dysfunction (POCD) affect the survival rate and quality of life of patients after liver transplantation. Dexmedetomidine may reduce the occurrence of POCD, and resting state functional magnetic resonance imaging (rs-fMRI) is the latest imaging method to study brain structure and function. It has not been reported that the relationship between the effects of dexmedetomidine on postoperative cognitive function and resting state functional magnetic resonance imaging in liver transplantation patients. This study is to observed the effect of dexmedetomidine on postoperative cognitive function, serum markers of brain injury and the spatial connection and function of brain resting state in patients with liver transplantation, and to explore the possible central mechanisms, so as to provide a basis for optimizing clinical pathways. |

药物成份或治疗方案详述: |
患者符合入选条件,患者入手术室后常规监测心电图、脉搏氧饱和度、血压,局麻下桡动脉穿刺置管监测有创动脉血压。AspectA-2000BIS监测仪(美国)连续监测BIS。麻醉诱导为咪达唑仑0.05 mg/kg, 舒芬太尼0.5μg/kg,依托咪酯0.3 mg/kg,罗库溴铵0.9 mg/kg,气管内插管机械通气,吸入空气和氧气混合气,呼气末正压5cmH2O,维持呼气末二氧化碳分压(PetCO2)35-45 mmHg。麻醉维持为静脉持续泵注丙泊酚、舒芬太尼0.3-0.5μg/kg–1/h–1、罗库溴铵0.3-0.5 mg/kg–1/h–1,调节丙泊酚输注速度,维持BIS值在35-45之间。 常规麻醉加右美托咪定组,诱导前给予右美托咪定0.5μg/kg,10分钟内泵注完毕,以后以0.5μg?kg–1/h–1 维持量持续泵注至手术结束。 |

Description for medicine or protocol of treatment in detail: |
Patients met the enrollment conditions. After patients entered the operating room, ECG, pulse oximetry, and blood pressure were routinely monitored. Invasive arterial blood pressure was monitored by radial arterial puncture under local anesthesia. BIS value was measured continuously by the AspectA-2000 (USA). Anesthesia was induced by midazolam 0.05 mg/kg, sufentanil 0.5 μg/kg, etomidate 0.3 mg/kg and rocuronium 0.9 mg/kg. Patients were given tracheal intubation and mechanical ventilation with inhalation of air and oxygen gas,positive end expiratory pressure 5cmH2O, and end-tidal carbon dioxide partial pressure (PetCO2) 35-45 mmHg. The anesthesia was maintained by continuous intravenous pumping of propofol, sufentanil 0.3-0.5μg/kg–1/h–1 and rocuronium 0.3-0.5 mg/kg–1/h–1. The infusion rate of propofol was adjusted to maintain BIS value between 35 and 45. In routine anesthesia plus dexmedetomidine group, dexmedetomidine 0.5μg/kg was intravenous injected before induction within 10 minutes, and then pumped with 0.5μg/kg–1/h–1 continuously until the end of surgery. |

排除标准: |
术前已出现肝性脑病(包括前驱期及昏迷前期)者;肝功能衰竭者;存在神经精神系统疾病者;服用精神类药物者;术前简易精神状况检查量表(MMSE)评分<24分;视力或听力障碍以及存在交流困难不能完成量表测试者;有药物滥用史者;房室传导阻滞(II 度以上)、病态窦房结综合征(包括植入心脏起搏器)等缓慢性心律失常者;体内有植入物等磁共振扫描的禁忌症者。 |

Exclusion criteria: |
Patients with hepatic encephalopathy (including period of prodromal and pre-coma) before surgery; those with liver failure;those with neuropsychiatric diseases; those taking psychotropic substances; patients with preoperative mini mental state examination (MMSE) scale score <24 points; those with vision or hearing impairment and patients having difficulty communicating and accomplishing the test; those with a history of drug abuse; patients with slow arrhythmia such as atrioventricular block (above II),sick sinus syndrome (including implantation of a pacemaker); patients with contraindications to magnetic resonance scans such as implants. |
