研究目的：

1、为PICS患者服务，良好运行ICU后门诊，促进全民“大健康”。 2、探讨与社区医疗服务中心合作的治疗管理模式，对ICU后综合征的影响。 3、完善ICU出院患者医疗数据库。 4、为国内同行提供PICS治疗管理经验，为管理者提供决策依据。  

Objectives of Study：

1. Serve for the patients with post-ICU syndrome, in order to run the post-ICU clinic well, and promote the "great health" of the whole people; 2. To explore the effect of treatment management mode in cooperation with community medical service centers on post-ICU syndrome; 3. Improving the medical database of ICU discharged patients; 4. Provide domestic counterparts with experience in the management of post-ICU syndrome treatment, and provide decision-making basis for managers.

排除标准：

2019年7月—2020年6月出我院ICU的存活者或家属不配合随访采集信息的。

Exclusion criteria：

will be excluded the survivors or family members of ICU discharged from our hospital from July 2019 to June 2020 who do not cooperate with follow-up to collect information.
