研究目的：

通过观察比较 ICU 机械通气患者正压气管拔管和负压气管拔管后，患者心率、血压及氧合的变化和呛咳、喉头痉挛、吸入性肺炎的发生率，为临床上 ICU 患者选择更佳的气管拔管方法。

Objectives of Study：

By observing and comparing the changes of heart rate, blood pressure and oxygenation and the incidence of cough, laryngeal spasm and aspiration pneumonia after tracheal extubation under positive pressure and negative pressure in ICU patients with mechanical ventilation, a better method of tracheal extubation can be selected for ICU patients in clinical practice.

纳入标准：

Inclusion criteria

排除标准：

1. 严重的肺部疾病者；
2. 有严重的神经系统功能障碍者；
3. 过度肥胖 BMI>40；
4. 声带麻痹；
5. 重症肌无力；
6. 妊娠。

Exclusion criteria：

1. Patients with severe lung disease;
2. Patients with severe nervous system dysfunction;
3. Obesity, BMI > 40;
4. Vocal cord paralysis;
5. Myasthenia gravis;
6. Pregnancy.

由课题负责人使用随机数字表法随机分组。在线随机数生成器生成1-999之间的随机数，按每行10个数摘抄成一个10行10列的数字表，以第3行第4列开始取数，共取180个数，按入组顺序分给受试对象，如受试对象为奇数则入正压拔管组，如为偶数，则入负压拔管组。
