Objectives of Study：

To evaluate the effect of comprehensive assessment and multi-disciplinary management model on the geriatric and frail patient

药物成份或治疗方案详述：

纳入标准：

Inclusion criteria

排除标准：

1.已参与其他类似干预试验；
2.近3月规律执行营养师/康复师/中医师开具的营养/康复/中医处方；
3.近3月规律每周进行3小时以上中等强度运动训练（最大心率达50%*（220-年龄））；
4.无法完成SPPB（研究的主要结果）；
5.无法独自参与并坚持运动；
6.长期居住医院或疗养院；
7.痴呆或严重认知障碍；
8.其他严重的共病状态损害患者能力；
9.各种疾病终末期，预期寿命＜12个月；
10.主要研究者或治疗医师认为排除的任何其他情况或事件。

Exclusion criteria：

1. Participated in other similar clinical trial;
2. Regular execution the prescription prescribed by dietitian / rehabilitator / Chinese medical practitioner in the last three months;
3. Regular conduction moderate intensity exercise training in the last three months (the maximum heart rate was 50% * (220-age);
4. Unable to complete the SPPB (main result of the study);
5. Unable to participate in and stick to exercise alone;
6. Residence in a hospital or sanatorium for a lang time;
7. Dementia or severe cognitive impairment;
8. Other severe concomitant conditions impair the ability of the patient;
9. The end-stage disease, and the life expectancy < 12 months;
10. Any other conditions or events that lead researchers or therapists consider to be excluded.
