研究目的:

高频震荡通气(HFOV)是治疗新生儿持续性肺动脉高压(PPHN)的重要通气模式之一。容量保证(VG)较传统压力限制通气(SIMV)相比,在新生儿呼吸衰竭的治疗中可以防止低碳酸血症,缩短机械通气时间。最新研究显示:VG能减少早产儿脑室内出血、脑室周围白质软化及支气管肺发育不良的发生率,对远期神经系统发育结局的影响尚不确切。本研究拟通过前瞻性随机对照研究比较VG联合HFOV在治疗PPHN中的近期通气效果及并发症和远期神经系统预后。  

Objectives of Study:

High frequency oscillatory ventilation(HFOV) is one of the most important modes of ventilation in the treatment of persistent pulmonary hypertension of the new-born(PPHN).Volume guarantee(VG) can prevent hypocapnia and shorten mechanical ventilation time in the treatment of neonatal respiratory failure compared with synchronized intermittent mandatory ventilation (SIMV). Recent studies have shown that VG can reduce the incidence of intraventricular hemorrhage, periventricular leukomalacia, and bronchopulmonary dysplasia in premature infants, however, the effect on the long-term outcome of nervous system development is still unclear.In this study, prospective randomized controlled studies were conducted to compare the short-term ventilation effects, complications and long-term neurological prognosis of VG combined with HFOV in the treatment of PPHN.

Inclusion criteria

排除标准:

排除标准:(1)先天性呼吸道畸形,如Pierre-Robin综合症,先天性喉软化等。(2)明显的先天性畸形或染色体疾病(完全性腭裂,21-三体综合症,18-三体综合征等);(3)入组前已有肺出血或肺气漏者。(4)复杂性先天性心脏病者。(5)III-IV级颅内出血者。(6)肺泡毛细血管发育不良并发PPHN者。

Exclusion criteria:

(1) Congenital respiratory malformations, such as Pierre-Robin syndrome, congenital laryngeal softening, etc.;
(2) obvious congenital malformations or chromosomal diseases (complete cleft palate, trisomy 21, trisomy 18, etc.);
(3) those who had pulmonary hemorrhage or air leakage before admission;
(4) Complex congenital heart disease;
(5) Class III-IV intracranial hemorrhage;
(6) Pulmonary alveolar capillary dysplasia complicated with PPHN.
