Objectives of Study：

To validate the effect of continuous care mode of type 2 diabetes mellitus based on APP "hospital-community" linkage.

纳入标准：

Inclusion criteria

排除标准：

1 研究对象在研究期间由于各种原因退出、失访、不配合、死亡等；
2 胰岛素泵使用者；
3 资料不全者；
4 严重糖尿病合并症者；
5 妊娠期或哺乳期者；

Exclusion criteria：

1. During the study period, the subjects quit, lost interviews, did not cooperate and died due to various reasons.
2. Insulin pump users;
3. Those with incomplete data;
4. Severe complications of diabetes mellitus;
5. Pregnancy or lactation.

随机方法（请说明由何人用什么方法产生随机序列）：

采用简单随机化的方法，将符合纳入标准的患者根据糖尿病就诊时间顺序依次编号为001-120，然后从随机数字表上抄录随机数字与之相对应，对随机数字进行排序，随机数字的序号为奇数者是对照组，随机数字的序号为偶数者是试验组。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Using a simple randomization method, the patients who met the inclusion criteria were numbered 001-120 according to the time sequence of diabetes treatment, and then the random numbers were transcribed from the random number table, and the random numbers were sorted.
