研究目的:

(1)利用已有的认知障碍队列,从氧化应激、神经细胞凋亡、糖代谢及糖基化修饰、tau蛋白的磷酸化、Aβ沉积以及脑白质损伤等方面进行脑脊液及血清学分析,探索并比较血管病后认知功能障碍和AD发病的机制之间的共性和特性,为建立脑血管病后认知障碍的预测模型提供理论基础; (2)新建缺血性脑血管病人群队列,采用前瞻性巢式病例对照设计,利用最新微量组学等技术研究轻型卒中、短暂性脑缺血发作、慢性灌注障碍等对于血管病后认知障碍病理学分子影像和外周标志物的影响,验证前述差异蛋白及基因预测脑血管病后认知损伤的准确度,建立脑血管病认知损伤的预测模型;  

Objectives of Study:

To explore and compare the commonness and characteristics of cognitive impairment and the pathogenesis of AD in the existing cognitive impairment cohort from oxidative stress, neuronal apoptosis, glycometabolism and glycosylation modification, tau phosphorylation, Aβ deposition and white matter injury, so as to establish a predictive model of cognitive impairment after cerebrovascular disease. Establish a new cohort of patients with ischemic cerebrovascular disease. A prospective nested case-control design will be used to study the effects of mild stroke, transient ischemic attack and chronic perfusion disorder on pathological molecular images and peripheral markers of cognitive impairment after vascular disease by using the latest microhistology techniques. To verify the accuracy of predicting cognitive impairment after cerebrovascular disease of the aforementioned differential proteins and genes.

纳入标准:

Inclusion criteria

排除标准:

(1)筛选前2周内使用过影响认知功能的药物;
(2)存在其他导致认知功能减退的中枢神经系统情况,如:代谢性脑病、帕金森综合征、亨廷顿病、硬膜下血肿、正常颅压脑积水、脑部肿瘤、脑外伤等;
(3)存在其他导致认知功能减退的全身性情况,如甲状腺功能减退、维生素B12或叶酸缺乏、烟酸缺乏、严重贫血、肝功能不全、肾功能不全、低钠血症、低钙血症、神经梅毒、HIV感染、酒精及药物滥用等;
(4)存在精神障碍如抑郁、精神分裂症等所导致的认知功能障碍;
(5)存在核磁禁忌及拒绝抽血者;
(6)存在严重心、肝、肾等重要器官疾病患者;
(7)存在视力障碍、失语、耳聋等不足以执行任务及量表操作;
(8)既往有脑卒中病史并有严重后遗症;
(9)罹患无法配合完成认知检查的疾病;
(10)拒绝签署知情同意书者。

Exclusion criteria:

(1) Drugs affecting cognitive function were used within 2 weeks before screening;
(2) There are other central nervous system conditions leading to cognitive impairment, such as metabolic encephalopathy, Parkinson's syndrome, Huntington's disease, subdural hematoma, normal intracranial pressure hydrocephalus, brain tumors, brain trauma, etc.;
(3) There are other systemic conditions leading to cognitive impairment, such as hypothyroidism, vitamin B12 or folic acid deficiency, nicotinic acid deficiency, severe anemia, liver insufficiency, renal insufficiency, hyponatremia, hypocalcemia, neurosyphilis, HIV infection, alcohol and drug abuse;
(4) Cognitive impairment caused by mental disorders such as depression and schizophrenia;
(5) those who refuse to take blood or who are contraindicated by nuclear magnetic resonance (NMR);
(6) patients with serious heart, liver and kidney diseases;
(7) Visual impairment, aphasia and deafness are not enough to perform tasks and scale operation;
(8) History of stroke and severe sequelae;
(9) suffering from illness that can not cooperate with the completion of cognitive examination;
(10) Those who refuse to sign the informed consent.
