研究目的：

（1）提出医院-社区-家庭-个体互动式的脑卒中健康管理综合干预模式； （2）设计和开发脑卒中健康管理综合干预APP； （3）通过随机对照试验评价干预效果，为我国脑卒中防控提供新的干预手段。  

Objectives of Study：

1. Propose an integrated intervention model for health management of stroke with hospital-community-family-individual interactions; 2. Design and develop comprehensive intervention APP for stroke health management; 3. Evaluate the effects of intervention model through parallel, and provide a new intervention method for community-based prevention and control of stroke in China.

纳入标准：

Inclusion criteria

排除标准：

（1）患有严重或威胁生命的疾病；
（2）短期居住的外地患者；
（3）试验过程中移居其他城市的患者或高危人群；
（4）患者或高危人群及其家属均无智能手机或不能独立完成APP基本操作；
（5）同期参加了其他相似干预项目。

Exclusion criteria：

1. The patients with serious or life-threatening diseases;
2. Short-term resident patients from other places;
3. Patients or high-risk groups who migrate to other cities during the project;
4. Patients or high-risk groups and their families do not have smartphones or can not independently complete the basic operation of smartphone APP;
5. Patients participate in similar intervention projects during the same period.

本研究采用多阶段分层整群随机抽样的方式，首先按地域和气候差异分为南方和北方，在南北方各抽取2个省，在每个省抽取符合现场备选和样本量的县区，然后以村和居委会为单位，将人群随机分配到干预组和对照组。

This study adopts a multi-stage stratified cluster random sampling method. We selected two provinces from the north and the south of China each, then choose eligible counties and districts from each province, and take the village and neighborhood committee as the unit to randomly assign the population.
