研究目的：

项目拟通过应用“云+端”的方式，基于健康云平台，糖尿病患者在社区卫生单位或家庭所测得多项糖尿病管理数据如血糖、血脂、膳食摄入、运动量、睡眠、体重变化等，将自动上传至数据中心，服务端利用数据进行糖尿病健康跟踪、预警、营养方案调整、运动方案调整以及双向转诊，期望通过采用“云+端”的方式对糖尿病患者加强营养和体重管理，以帮助糖尿病患者减少住院次数，减少服药量，延迟糖尿病并发症的发生或降低并发症的发生率，提高患者生活质量，促进糖尿病患者健康，减少国家卫生经济负担和糖尿病患者家庭经济负担。  

Objectives of Study：

Through the application of the "cloud + terminal" approach, based on the health cloud platform, diabetes patients have measured a number of diabetes management data such as blood sugar, blood fat, dietary intake, exercise, sleep, and weight changes in community health units or households. The data will be automatically uploaded to the data centre, and the service will use the data for diabetes health tracking, early warning, nutrition program adjustment, exercise program adjustment and two-way referral. It is hoped that the nutrition and weight management of diabetes patients will be strengthened through the use of the "cloud + terminal" approach. To help diabetes patients reduce the number of hospitalizations, reduce the amount of medication, delay the occurrence of diabetes complications or reduce the incidence of complications, improve the quality of life of patients, promote the health of diabetes patients, and reduce the National Health economic burden and the family economic burden of diabetes patients.

纳入标准：

Inclusion criteria

排除标准：

1. 甲亢;
2.恶性肿瘤等预期寿命短疾病患者;
3.糖尿病严重并发症;
4.各种严重的慢性疾病或肝肾功能不全;
5.曾有过严重的低血糖;
6.依从性差的患者（不能保证完成12个月的干预和期间的复诊回访）;
7. 行动不便，不能定期回访。

Exclusion criteria：

1. Hyperthyroidism;
2. Patients with short life expectancy such as malignant tumors;
3. Severe complications of diabetes;
4. All serious chronic diseases or hepatic and renal insufficiency;
5. Had severe hypoglycemia;
6. Patients with poor compliance(the completion of 12 months of intervention and follow-up visits during the period can not be guaranteed);
7. Inconvenient movement, can not return to visit regularly.
