排除标准：

1. 在外院有万古霉素使用治疗史；
2. 万古霉素血药浓度数值非稳态谷浓度者。

Exclusion criteria：

1. Patients with a therapeutic history of vancomycin;
2. Vancomycin TDM results were not steady trough concentration;
