药物成份或治疗方案详述: |
以3-12岁机器人辅助腹腔镜手术患儿为研究对象,开展前瞻性单中心随机对照临床研究,观察、比较ERAS管理与传统围手术期管理在机器人辅助下小儿腹腔镜肾盂成型手术中的安全性、有效性以及对患儿围手术期应激反应和院内成本的影响。收集申请人所在医院儿科收治的接受机器人辅助下腹腔镜肾盂成型手术治疗的3-12岁小儿286例。随机分为ERAS管理组143例和传统围手术期管理组143例。ERAS组采取术前宣教、术前缩短禁食时间、术前不常规胃管、术前安抚、术中保温、避免使用吸入麻药、术中限制输液、切口处理、不常规留置引流管、术后多模式镇痛、术后尽早活动、术后尽快进食等干预措施,对照组不做特殊处理。通过统计学方法分析,ERAS和传统围手术期管理不同的处理方式对患儿术后并发症发生的影响、炎症因子表达的影响以及对总体并发症发生率、术后住院时间、住院费用和家属满意度等,明确①ERAS的安全有效性,②ERAS对机器人辅助小儿腹腔镜肾盂成型手术中患儿围术期应激反应的影响,③ERAS对腹腔镜手术患儿院内成本的影响。 |

Description for medicine or protocol of treatment in detail: |
In 3 to 12 years old children with robotic assisted laparoscopic surgery as the research object, and carry out prospective single-center randomized controlled clinical research, observation and comparison of the ERAS with traditional perioperative management in the robot assisted pediatric laparoscopic renal pelvis molding operation safety, efficacy and the children of perioperative stress response and the impact of hospital cost.A total of 286 cases of children aged 3-12 years who received robot-assisted laparoscopic pyeloplasty in the department of pediatrics of the hospital where the applicant was admitted were collected.143 cases were randomly divided into ERAS management group and traditional perioperative management group.ERAS group adopt preoperative education, preoperative fasting time, preoperative routine gastric tube, preoperative calm, intraoperative heat preservation, avoid the use of inhaled anesthesia, intraoperative infusion, processing of the incision, no routine indwelling drainage tube, multimodal analgesia after operation, postoperative activities as soon as possible, as soon as possible after eating intervention measures, such as the control group does not do special treatment.ERAS through the analysis of statistical methods and traditional perioperative management of different processing methods on postoperative complications, the influence of inflammatory factor expression and the influence of the overall incidence of complications, length of hospital stay, postoperative hospitalization expenses and family satisfaction, such as clear (1) the security effectiveness of ERAS, (2) the ERAS of robot assisted pediatric laparoscopic renal pelvis moulding children perioperative stress response in the operation, the influence of (3) ERAS of laparoscopic surgery in children with the influence of hospital cost. |

年龄<3岁或年龄> 12 岁; 既往有腹部手术史; 合并穿孔 梗阻 严重感染 中重度脱水 中毒性休克 高度腹胀 重度营养不良 高热; 合并神经系统 免疫系统疾病 严重心 肝 肾功能异常以及恶性肿瘤者; 有麻醉 气腹等禁忌症者; 家属不依从或不能完成本课题涉及指标测定者 |
