探讨急性心肌梗死后左室游离壁破裂非外科治疗的策略及其可行性。

Objectives of Study：

This study sought to explore the optimal non-surgical treatment strategy and its feasibility for the patients of LVFWR post AMI.

Inclusion criteria

AMI is diagnosed according to at least two of the following three criteria: ischemic chest pain or chest tightness lasting for more than 30 min; Sublinguinal administration of nitroglycerin showed no remission, and ST segment elevation of two consecutive leads (I,II,III, aVL, aVF leads ≥1.0mm or chest leads V1~V6≥ 2.0mm), and myocardial enzyme level increased by more than 2 times.
The diagnostic criteria for LVFWR were progressive exacerbation of heart failure or sudden electromechanical separation, slow ventricular rate, acute jugular venous dilation, and cyanosis of the head and neck.Patients without electro-mechanical separation had acute non-arrhythmic hypotension (systolic blood pressure ≤80mm Hg). Ultrasound showed a rapid increase in newly developed pericardial effusion or pericardial effusion in the original pericardial effusion with or without cardiac tamponade, and doppler examination confirmed the presence of ventricular wall insufficiency with flow signals passing through the free wall. Subacute LVFWR depends on pericardial effusion with or without pseudoaneurysm and mild or severe cardiac tamponade. Doppler examination confirmed the presence of incomplete ventricular free wall structure and transmural blood flow signals. According to ultrasonic changes, it can be divided into two types: outflow type and oozing type.
