研究目的：

以妊娠糖尿病每日多次皮下注射胰岛素治疗为对照，评价采用贴敷式胰岛素泵治疗妊娠糖尿病的有效性和安全性。

Objectives of Study：

The study is performed to evaluate the efficacy and safety of adhesive insulin pumps in the treatment of gestational diabetes by taking multiple daily subcutaneous insulin injections as control.

药物成份或治疗方案详述：

采用多中心、开放试验设计；研究周期自孕期需要胰岛素治疗开始至产后。经过筛选检查符合入选标准的妊娠糖尿病160例将按照1:1的比例自愿进入对照组或试验组，对照组接受每日多次（≥2次）皮下胰岛素注射（MSII），实验组接受贴敷式胰岛素泵(CSII)治疗，治疗期间为整个孕期。依据患者开始接受治疗的孕周，安排受试者应用胰岛素后每隔4周进行电话或微信或APP随访，指导患者调整胰岛素剂量。

纳入标准：

Inclusion criteria

1. Be able to understand the purpose of the clinical trial, voluntarily participate and have signed an informed consent form;
2. Aged 18 to 45 years old;
3. Body mass index (BMI) at 18.0-38.0 kg/m2;
4. Pre-pregnancy type 2 diabetes with pregnancy: Type 2 diabetes pregnancies that are clearly diagnosed according to WHO diagnostic criteria;
5. Gestational Diabetes: According to the 2017 Chinese Diabetes Care Guidelines:
It refers to mild abnormal glucose metabolism during pregnancy, but blood glucose does not reach the level of dominant diabetes, excluding diabetes present before pregnancy. 75gOGTT: FPG≥5.1~<7.0mmol/L, blood glucose ≥10.0mmol/L 1 hour after OGTT, 8.5~<11.1mmol/L 2 hours after OGTT, an abnormality is diagnosed as gestational diabetes;
6. Dominant Diabetes in Pregnancy: According to the 2017 Chinese Diabetes Mellitus Guidelines for Diagnosing Type 2 [6]: Diagnostic criteria for pregnancy found in non-pregnant populations: FPG ≥ 7.0 mmol/L or 2 hours after OGTT ≥ 11.1 mmol/L, or random blood glucose ≥ 11.1 mmol/L. There are no more than three symptoms and repeated measurements on different days, there is still a blood glucose standard;
7. Willing and able to use the meter to monitor fingertip blood glucose and fill out the patient diary card as required;
8. Ability to follow the insulin treatment plan and dietary guidance determined by the investigator.

排除标准：

1. 已知对胰岛素、胰岛素类似物或其制剂中的成分过敏；
2. 筛选时转氨酶（ALT/AST）大于正常值上限的2.5倍；
3. 筛选时血清肌酐（Cr）大于正常值上限1.5倍；尿常规检查中的尿蛋白≥+2;
4. 高血压：经治疗无法控制或未经治疗过的重度高血压，定义为筛选时收缩压≥150mmHg和/或舒张压≥100mmHg；
5. 除外影响糖代谢相关疾病；
6. 研究医生判断患者依从性差无法按照要求完成研究、或其它原因不适合参加。

Exclusion criteria：

1. Known to be allergic to insulin, insulin analogues or ingredients in their preparations;
2. Transaminase (ALT/AST) is greater than 2.5 times the upper limit of normal values during screening;
3. Serum creatinine (Cr) at screening is greater than 1.5 times the upper limit of normal; urine protein in urine routine examination ≥ +2;
4. Hypertension: Severe hypertension that cannot be controlled or treated without treatment, defined as systolic blood pressure ≥150 mmHg and/or diastolic blood pressure ≥100 mmHg at screening;
5. Except for diseases affecting glucose metabolism;
6. The research doctor judges that poor patient compliance does not complete the study as required, or that other reasons are not suitable for participation.
