研究目的：

了解社区冠心病患者自我管理行为、生活质量现状，探讨基于网络互动平台的冠心病患者自我管理模式干预对冠心病患者自我管理行为、生活质量的影响，从而为完善冠心病管理，提升患者生存质量提供依据。  

Objectives of Study：

This study aims to investigate the self-management behavior, quality of life among patients with coronary heart disease (CHD) in community. Futher, the effectiveness of web-based self-management intervention on the self-management behavior and health-related quality of life of patients with CHD were explored. Thus, we were able to provide scientific evidence for the promotion of self-management mode in community.

Inclusion criteria

排除标准：

（1）患者具有严重的精神疾患；
（2）病情严重，无法生活自理者。

Exclusion criteria：

(1) The patient has a serious mental illness;
(2) The condition is serious and cannot be taken care him/herself.

为避免沾染，研究者采用单纯随机抽样的方法，确定A社区卫生服务中心为网络平台干预组的研究现场；B社区卫生服务中心为对照组和自我管理组的研究现场。其中，B社区卫生服务中心辖区内下属共有12个社区，将12个社区编号，采用单纯随机抽样方法，选取其中6个社区为对照组的研究现场，另外6个为自我管理组的研究现场。确定三个组的研究现场后，根据纳入标准和排除标准，在各自的研究现场选取符合标准的研究对象。
