研究目的：

本研究基于WHO提出的创新性慢性病管理框架（ICCC）为理论基础，构建慢性病患者智慧健康管理模式并试点应用。具体目标包括：1）评估慢性病患者生活质量、自我管理行为、自我效能及健康管理需求；2）发展慢性病患者智慧健康管理模式草案；3）搭建慢性病患者智慧健康管理平台；4）采用随机对照研究在西安试点应用，以RE-AIM理论框架评价慢性病患者智慧健康管理模式的效能。以期为探索适合我国国情的“广覆盖、高效益、低成本”的慢性病健康管理模式提供新的思路，亦为政府制定相关的卫生政策提供参考。  

Objectives of Study：

The goal of this program is to develop, deliver and evaluate smart health management for patients with chronic illness, based on innovative chronic disease management framework (ICCC) proposed by WHO. Specific objectives include: 1) To assess the quality of life, self-management behaviors, self-efficacy and health management needs of patients with chronic illness; 2) To develop smart health management protocols for patients with chronic illness; 3) To establish smart health management platform with the assistance of IT specialist4) To evaluate the effectiveness and efficiency of smart health management for patients with chronic illness based on the RE-AIM theoretical framework. Moreover, we can generate evidence for informing policy to improve chronic illnesses management by demonstrating Broad Coverage, Low costs and High Efficiency.
