研究目的:

重复经颅磁刺激(rTMS)是促进患者运动功能康复的重要手段,但其对神经重塑的具体调控机制尚不清楚。我们采用多模态磁共振影像技术定量化反映卒中后脑结构保留度,后予以不同方式的rTMS刺激,探寻不同rTMS刺激方案对卒中后不同结构保留度的患者的运动功能恢复的差异,通过多模态磁共振影像技术探究其中的恢复机制,为多模态影像技术指导卒中后rTMS个体化治疗提供理论基础。  

Objectives of Study:

Repeated transcranial magnetic stimulation (rTMS) is an important means to promote the rehabilitation of motor function in patients, but its specific regulatory mechanism for nerve remodeling is still unclear. We used multi-modal magnetic resonance imaging technology to quantitatively reflect the brain structure retention after stroke, and then gave different rTMS stimulation methods to explore the difference of motor function recovery between different rTMS stimulation schemes for patients with different structure retention after stroke. We used multi-modal magnetic resonance imaging technology to explore the recovery mechanism, and to guide the individualized treatment of rTMS after stroke by multi-modal imaging technology. Therapy provides a theoretical basis.

纳入标准:

Inclusion criteria

排除标准:

①脑外伤史、精神病史;②合并严重心、肝、肾功能不全;③合并肌源性疾病或(和)周围神经系统疾病;④意识障碍,不能配合检查;⑤存在MR检查禁忌症;⑥既往有脑出血、蛛网膜下腔出血、脑肿瘤、脑外伤及其他神经精神系统病史;⑦发病前存在肢体运动功能障碍;⑧存在癫痫发病史。

Exclusion criteria:

(1) History of brain trauma and psychosis;
(2) Serious heart, liver and kidney dysfunction;
(3) Myogenic diseases or (and) peripheral nervous system diseases;
(4) Consciousness disorders, unable to cooperate with the examination;
(5) Taboos of MR examination;
(6) History of cerebral hemorrhage, subarachnoid hemorrhage, brain tumors, brain trauma and other neuropsychiatric diseases;
(7) Existence of limb motor function before onset; Dysfunction; Epilepsy history.

将每个受试者所采用的刺激方案分装入一个信封内,则在信封外编序号(1,2,3,……);然后,按照随机分配表上确定2个分组(FA>0.5和FA<0.5)的顺序号抽取不同的刺激方案,使得其最终随机分为4个组。

Blinding:

The stimulus program used by each participant was packed into an envelope and numbered (1, 2, 3,...) outside the envelope. Then, according to the sequence number of two groups (FA > 0.5 and FA < 0.5) determined on the random allocation table, different stimulus schemes were extracted, so that they were eventually randomly divided into four groups.
