脑卒中是导致成人残疾的重要原因之一,运动功能障碍是其常见后遗症。卒中后运动恢复与大脑皮质功能重组密切相关,如何客观评估脑功能重塑过程是康复研究的重要问题。rTMS等非侵入性脑刺激技术已广泛应用于卒中后功能恢复,但由于刺激部位、参数、病程阶段及结局指标不同,其疗效仍存在异质性。fNIRS作为一种无创、便携且适用于运动任务监测的脑功能成像技术,可通过检测氧合血红蛋白和脱氧血红蛋白变化反映皮质激活状态,更适合康复场景下的重复测量。当前,TMS已常用于脑卒中后运动功能障碍康复,但其治疗前后脑皮质变化特征尚不明确。采用fNIRS在TMS治疗前后进行评估,观察皮质模式变化并分析其与运动功能变化的关系,有助于从脑功能层面解释TMS治疗相关变化,为卒中康复疗效评估提供客观依据。 1.主要目的: 观察脑卒中后运动功能障碍患者在TMS治疗前后fNIRS皮质特征的变化。 2.次要目的: 分析TMS治疗前后fNIRS指标变化与运动功能量表变化之间的关系,探索fNIRS作为卒中后运动康复客观评估工具的应用价值。 |

Objectives of Study: |
Stroke is one of the leading causes of adult disability, with motor dysfunction being a common sequela. Post‑stroke motor recovery is closely associated with cortical reorganization. How to objectively evaluate the process of brain functional remodeling is a critical issue in rehabilitation research. Non‑invasive brain stimulation techniques such as repetitive transcranial magnetic stimulation (rTMS) have been widely used for functional recovery after stroke. However, due to variations in stimulation sites, parameters, disease stages, and outcome measures, the therapeutic effects remain heterogeneous. Functional near‑infrared spectroscopy (fNIRS) is a non‑invasive, portable, and task‑suitable brain imaging technique that can reflect cortical activation status by measuring changes in oxy‑ and deoxy‑hemoglobin concentrations, making it particularly suitable for repeated measurements in rehabilitation settings. Currently, TMS is commonly applied in the rehabilitation of post‑stroke motor dysfunction, but the cortical changes before and after treatment are not yet well characterized. Using fNIRS to assess cortical activation patterns before and after TMS treatment, and analyzing their relationship with changes in motor function, may help elucidate the neural mechanisms underlying TMS‑related improvements and provide objective evidence for evaluating stroke rehabilitation outcomes. 1. Primary objective: To observe the changes in cortical fNIRS characteristics in patients with motor dysfunction following a stroke, before and after TMS treatment. 2. Secondary objectives: To analyze the relationship between changes in fNIRS indicators before and after TMS treatment and those in motor function scales, thereby exploring the potential of fNIRS as an objective tool for assessing motor rehabilitation after a stroke. |
