研究目的：

以H型高血压合并脑卒中患者为研究人群，通过多中心随机对照试验，验证家庭参与式数字化健康管理平台延续护理对提高脑卒中患者的健康行为水平以及降低脑卒中复发率的效果。  

Objectives of Study：

The study is a multi-center, randomized controlled trial to determine whether the intervention of family participatory digital health management platform will improve the health behavior and reduce the rate of stroke recurrence in ischemic stroke patients with type H hypertension.

纳入标准：

Inclusion criteria

排除标准：

（1）继发性高血压。
（2）合并有严重肾和肝脏疾病或恶性肿瘤或精神疾病。

Exclusion criteria：

(1) Secondary Hypertension;
(2) History of severe kidney or liver diseases , any known malignancy or mental disease.

1.由研究者采用电脑生成的随机数字随机分为对照组和干预组。 2.按患者出院顺序整群抽样，采用随机分组方法。选择符合纳入条件的患者，按出院顺序，根据患者的脑卒中发生的次数（首发或复发）进行分层随机分组。

The random allocation sequence will be computer generated in stratified for stroke times (recurrent or not) by the researcher who is not involved in the randomization. Patients will be randomised to either usual care or intervention group according to the patient discharge order.
