研究人工全膝、髋关节置换手术后使用利伐沙班与阿司匹林进行血栓预防的疗效差异

Objectives of Study：

to observe the difference of Aspirin and Rivaroxaban for venous thromboembolism prophylaxis after total knee or hip arthroplasty

药物成份或治疗方案详述：

纳入标准：

Inclusion criteria

排除标准：

(1)使用阿司匹林进行三级预防患者或双抗血小板治疗患者，包括既往3个月内有急性冠脉综合征病史的患者，冠状动脉支架植入术后患者，人工瓣膜置换术患者，或其他高血栓风险患者；
(2)VTE发生高风险患者，包括既往VTE病史，合并恶性肿瘤，合并高凝倾向患者，已知易栓情况，目前合并接受抗凝治疗的疾病；
(3)患者合并活动性溃疡，不能接受口服非甾体药物治疗，或者患者对口服阿司匹林不能耐受。
(4)合并其他心、脑、肺、肾、血液等其他严重器官疾病；
(5)有消化道出血病史或入院常规检查大便OB试验≥3次阳性者；
(6)凝血功能障碍、出血倾向或术前有活动性出血患者。
(7)全身动脉狭窄的病人或者因此行支架手术的患者。
(8)因股骨颈骨折行全髋关节置换的患者。
(9)肾功能不全患者。

Exclusion criteria：

1. patient who use aspirin for tertiary prevention or double antiplatelet therapy, including acute coronary syndrome in past 3 months, post-operation of PCI or cardiac valve replacement, and other high-risk patient;
2. patient with high risk of VTE, including previous VTE history, malignant tumor, hypercoagulable state, other known states that easily lead to thrombosis and undergoing anticoagulation;
3. patient with active ulcer that can not receive aspirin therapy or has aspirin intolerance;
4. patient with other severe disease in heart, brain, lung, kidney or blood;
5. patient with gastrointestinal hemorrhage history or OB test positive ≥3 times after admission;
6. patient with coagulation dysfunction, trend of hemorrhage or has active hemorrhage before operation;
7. patient with systematic artery stenosis or received stent implantation for such disease;
8. patient receiving total hip arthroplasty for femoral neck fracture;
9. patient with renal insufficiency.
