研究目的：

比较不同入路（前路、后路、前后联合）及不同术式（椎间盘切除融合、椎体次全切除融合、椎管扩大成形、椎板切除融合、人工间盘置换等）对颈椎椎旁肌横截面积（CSA）萎缩率及脂肪浸润（FI）增加率的影响，明确各术式对椎旁肌的保护与损伤模式，为临床术式选择及术后康复提供影像学依据。  

Objectives of Study：

To compare the effects of different surgical approaches (anterior, posterior, combined) and techniques (ACDF, ACCF, laminoplasty, laminectomy with fusion, artificial disc replacement, etc.) on the cross-sectional area (CSA) atrophy rate and fatty infiltration (FI) increase rate of cervical paraspinal muscles, and to identify the patterns of muscle protection or injury associated with each procedure, thereby providing imaging-based evidence for surgical decision-making and postoperative rehabilitation.

纳入标准：

Inclusion criteria

排除标准：

1. 合并颈椎感染、结核、原发性颈椎肿瘤（转移瘤可酌情纳入）； 2. 既往有颈椎手术史； 3. 术后MRI图像质量不佳，无法清晰勾画肌肉轮廓； 4. 严重颈椎畸形或创伤导致椎旁肌解剖结构严重破坏者； 5. 左利手患者（为控制手性对肌肉发育不对称的潜在影响）。

Exclusion criteria：

1. Inclusion of cervical spine infections, tuberculosis, and primary cervical tumors (metastases can be included as appropriate); 2. History of previous cervical spine surgery; 3. Poor quality of postoperative MRI images, making it impossible to clearly delineate muscle contours; 4. Severe cervical spine deformity or trauma resulting in severe disruption of the paravertebral muscle anatomical structure; 5. Left-handed patients (to control for the potential influence of handedness on muscle development asymmetry).

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

本研究采用纸质版病例报告表（CRF）进行数据采集，内容包括：患者基本信息（年龄、性别、BMI等）、手术信息（术式、节段、手术时间等）、影像学参数（术前及术后椎旁肌CSA、FI等）。CRF由两名研究者独立录入Excel数据库，并进行交叉核对。所有电子数据存储于医院加密科研云盘及专用电脑，仅项目组成员可访问。数据管理遵循医院科研数据管理规定，保存至研究结束后5年。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Paper Case Record Form (CRF) will be used to collect data including patient demographics, surgical information, and imaging parameters (CSA, FI). Two independent researchers will enter data into Excel and cross-check. All electronic data will be stored on encrypted institutional cloud storage and a dedicated computer, accessible only to the research team. Data will be retained for 5 years after study completion.
