研究目的：

胫神经脉冲射频治疗对糖尿病周围神经病变下肢疼痛的有效性研究，以6个月VAS下降≥50%为主要终点  

Objectives of Study：

Study on the efficacy of tibial nerve pulsed radiofrequency treatment for lower limb pain in diabetic peripheral neuropathy, with a primary endpoint of a ≥50% decrease in VAS at 6 months

纳入标准：

Inclusion criteria

排除标准：

1.其他病因所致神经病变：颈椎病、腰椎间盘突出症、椎管狭窄等引起的根性神经病变；腕管综合征、肘管综合征等卡压性周围神经病变；酒精性、尿毒症性、遗传性、炎症性（如吉兰-巴雷综合征）等其他原因所致的周围神经病变。
2.治疗前存在穿刺部位局部感染（皮肤破溃、红肿、脓性分泌物）；血小板计数<50×10^9/L，或INR>1.5，或正在服用抗凝药物（阿司匹林、氯吡格雷、华法林等）且未按操作规范停药；
3.合并下肢动脉硬化闭塞症（踝肱指数<0.9）或其他明显影响足部血供的血管疾病；
4.严重全身性疾病：严重心功能不全（NYHA III-IV级）、肝功能不全（ALT/AST >正常上限3倍）、肾功能不全（eGFR <30 mL/min/1.73m²）、恶性肿瘤（活动期或近5年内）、自身免疫性疾病活动期。
5.关键变量（基线VAS、随访VAS、治疗分组信息）缺失，或病历记录严重不全（缺失率>30%）；
6.精神或认知障碍：严重精神疾病（精神分裂症、双相情感障碍等）、认知功能障碍（MMSE<24分）导致无法配合评估或随访者。
7.妊娠或哺乳期：妊娠、哺乳期或有近期生育计划者；
8.治疗期间参与过其他干预性临床试验；

Exclusion criteria：

1. Peripheral neuropathy caused by other etiologies: radicular neuropathy caused by cervical spondylosis, lumbar disc herniation, spinal canal stenosis, etc.; compressive peripheral neuropathy such as carpal tunnel syndrome and cubital tunnel syndrome; peripheral neuropathy caused by other factors such as alcohol, uremia, hereditary, inflammatory (e.g., Guillain-Barré syndrome), etc. 2. Local infection at the puncture site before treatment (skin breakdown, redness and swelling, purulent discharge); platelet count <50×10^9/L, or INR >1.5, or currently taking anticoagulants (aspirin, clopidogrel, warfarin, etc.) without following the operation protocol to stop the medication. 3. Complicated with lower limb arteriosclerosis obliterans (ankle-brachial index <0.9) or other vascular diseases significantly affecting blood supply to the foot. 4. Severe systemic diseases: severe heart failure (NYHA III-IV), liver dysfunction (ALT/AST >3 times the upper limit of normal), renal dysfunction (eGFR <30 mL/min/1.73 m^2), malignant tumor (active or within the past 5 years), active autoimmune disease. 5. Missing key variables (baseline VAS, follow-up VAS, treatment group information), or severely incomplete medical records (missing rate >30%). 6. Mental or cognitive impairment: severe mental illness (schizophrenia, bipolar disorder, etc.), cognitive dysfunction (MMSE <24 points) leading to inability to cooperate with assessment or follow-up. 7. Pregnancy or breastfeeding: pregnant, breastfeeding, or planning recent conception. 8. Participation in other interventional clinical trials during the treatment period.
