研究目的:

本研究为探索性、前瞻性、观察性研究,主要目的为构建基于多靶器官微量多组学特征的cGVHD诊断模型。次要目的为探索基于移植后早期(如+100天,+180天)分子特征的cGVHD发生风险预测模型。同时,分析关键标志物涉及的核心生物学通路,探索cGVHD发生机制与新的治疗靶点。  

Objectives of Study:

This study is an exploratory, prospective, and observational research aimed at constructing a cGVHD diagnostic model based on multi-target organ micro multi omics features. The secondary objective is to explore a cGVHD risk prediction model based on early post transplant (e.g.+100 days,+180 days) molecular features. At the same time, analyzing the core biological pathways involved in key biomarkers, exploring the mechanism of cGVHD occurrence and new therapeutic targets.

Inclusion criteria

排除标准:

1. 严重并发症或高风险因素:活动性出血性疾病(如凝血功能障碍,国际标准化比值 INR>1.5,或血小板计数<60×10^9/L); 2. 近期(如 4 周内)发生严重感染(如肺炎、败血症),尚未控制; 3. 全身状态差:ECOG 评分>=3 或预计生存期<3 个月;心肺功能严重受损(如 NYHA 心功能分级 III 或 IV); 4. 合并其他恶性肿瘤,并且在进行治疗,或既往 5 年内罹患过其他恶性肿瘤的患者; 5. 存在脑功能紊乱的临床症状或严重的精神性疾病不能理解或遵从研究方案; 6. 具有严重的急性过敏反应的患者; 7. 研究人员认为其他原因不适合临床试验者,包括:前 4 周内进行外科手术尚未完全恢复的患者;筛选时患有癫痫或使用精神药物、镇静药物的患者;经药物治疗无法控制的糖尿病(空腹血糖>8.9 mmol/L);患有高血压且经两种或两种以下降压药治疗无法下降到以下范围内者(收缩压<160 mmHg,舒张压<100 mmHg);ALT 和 AST>2.5×ULN;DBIL 和 TBIL>2.0×ULN;血清肌酐>1.5×ULN;使用抗凝血或血小板功能的药物治疗(低分子量肝素除外)。 8. 无法按照研究者的要求完成研究。

Exclusion criteria:

1. Severe complications or high-risk factors: active bleeding disorders (such as coagulation dysfunction, International Normalized Ratio [INR] > 1.5, or platelet count < 60×10^9/L); recent severe infections (such as pneumonia or sepsis) occurring within the past 4 weeks that remain uncontrolled; 2. Poor general condition: Eastern Cooperative Oncology Group (ECOG) performance status >= 3 or expected survival < 3 months; severe impairment of cardiopulmonary function (such as New York Heart Association [NYHA] functional class III or IV); 3. Patients with concurrent other malignancies currently undergoing treatment, or those who have had other malignancies within the past 5 years; 4. Presence of clinical symptoms of brain dysfunction or severe psychiatric disorders that prevent understanding or compliance with the study protocol; 5. Patients with severe acute allergic reactions; 6. Other reasons deemed by investigators to make patients unsuitable for clinical trials, including: patients who have undergone surgery within the previous 4 weeks and have not fully recovered; patients with epilepsy or using psychotropic or sedative medications at screening; diabetes mellitus uncontrolled by medication (fasting blood glucose > 8.9 mmol/L); hypertension that cannot be reduced to the following range despite treatment with two or fewer antihypertensive agents (systolic blood pressure < 160 mmHg, diastolic blood pressure < 100 mmHg); alanine aminotransferase (ALT) and aspartate aminotransferase (AST) > 2.5 × upper limit of normal (ULN); direct bilirubin (DBIL) and total bilirubin (TBIL) > 2.0 × ULN; serum creatinine > 1.5 × ULN; use of anticoagulant or antiplatelet medications (except low-molecular-weight heparin). 7. Inability to complete the study as required by the investigators.
