研究目的:

本研究旨在依托多中心回顾性病例队列,构建高质量牙周病多模态数据库,整合临床参数、影像学指标、唾液 / 龈沟液生物标志物三类数据;通过单模态预训练、跨模态对齐技术,建立面向影像与临床指标的牙周疾病多模态大模型;最终为开发具备高临床实用性的牙周病 AI 多模态融合诊断模型、实现牙周病自动识别、精准分期分级奠定基础,推动牙周病诊疗向精准化、普惠化发展。  

Objectives of Study:

This study aims to establish a high-quality multimodal database for periodontal diseases based on a multicenter retrospective case cohort, integrating three types of data including clinical parameters, imaging indicators, and biomarkers in saliva and gingival crevicular fluid. By adopting unimodal pre-training and cross-modal alignment technologies, a multimodal large model for periodontal diseases targeting imaging and clinical indicators will be constructed. Ultimately, this research lays a foundation for developing clinically practical AI multimodal fusion diagnostic models for periodontal diseases to realize automatic identification and accurate staging and grading of the disease, and promotes the development of periodontal diagnosis and treatment toward precision and universal accessibility.

Inclusion criteria

排除标准:

1.纳入时间节点处于妊娠期内; 2.接受过正畸或正颌治疗; 3.纳入时间节点口腔存留牙≤10 颗。

Exclusion criteria:

1.Pregnancy at the time of enrollment;
2.History of orthodontic or orthognathic treatment;
3.Fewer than or equal to 10 remaining natural teeth at the time of enrollment.

由两名及以上高年资牙周专科医师依据 2018 年国际牙周病分类标准独立做出的牙周病诊断、分期及分级结果,意见不一致时经讨论达成共识。

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):

Periodontal diagnosis, staging, and grading independently determined by two or more senior periodontists according to the 2018 international classification, with consensus reached by discussion in case of disagreement.

2016 年 1 月 1 日至 2026 年 4 月 30 日在北京大学口腔医院、北京口腔医院、北京朝阳医院就诊,年龄 18–60 周岁、确诊为牙周病且临床与影像资料完善的患者。

Target condition (The target condition is a particular disease or disease stage that the index test will be intended to identify. Please specify the characteristics in detail; the population should has a complete spectrum and good representative):

Patients aged 18 to 60 years old who received treatment at Peking University School and Hospital of Stomatology, Beijing Stomatological Hospital and Beijing Chaoyang Hospital from January 1, 2016 to April 30, 2026, were diagnosed with periodontal disease and had complete clinical and imaging data.

容易混淆的疾病人群(即与目标疾病不易区分的一种或多种不同疾病,应避免采用正常人群对照的病例-对照设计):

非菌斑性牙龈炎(遗传性龈纤维瘤病等)、创伤性牙龈损伤、龈乳头炎等。
