研究目的：

1. 明确血浆同型半胱氨酸(Hcy)水平与脑小血管病(CSVD)患者淡漠症状严重程度之间的关系； 2. 验证CSVD影像学总负荷评分在Hcy与淡漠症状关联中的中介效应及其贡献度； 3. 为理解Hcy通过CSVD脑结构损伤导致淡漠的神经病理机制提供影像学依据。本研究旨在揭示"Hcy→CSVD→淡漠"的潜在病理通路，为临床干预提供新靶点。

Objectives of Study：

1. To determine the association between plasma homocysteine (Hcy) level and the severity of apathy in patients with cerebral small vessel disease (CSVD); 2. To validate the mediating effect and quantify the contribution of the total CSVD neuroimaging burden score in the relationship between Hcy and apathy; 3. To provide neuroimaging evidence for the neuropathological mechanism by which Hcy leads to apathy through CSVD-related structural brain damage. This study aims to reveal the potential pathological pathway of "Hcy→CSVD→apathy" and to provide new targets for clinical interventions.

纳入标准：

Inclusion criteria

排除标准：

1、由大血管病变（如颈内动脉、大脑中动脉重度狭窄或闭塞）、颅内占位、脑外伤、帕金森病、阿尔茨海默病或其他明确病因所致的痴呆。 2、有临床诊断的抑郁症、精神分裂症、双相情感障碍等精神疾病史，或正在服用抗精神病药物。 3、存在严重失语、失聪、视力障碍，无法完成神经心理学评估。 4、合并严重肝肾功能不全（如终末期肾病）、恶性肿瘤等影响生存期的大躯体疾病。 5、有 MRI 检查禁忌症（如心脏起搏器、金属植入物等）。 6、无法获取入组前 3 个月内血清同型半胱氨酸（Hcy）检测结果，或头颅 MRI 影像资料不完整（缺 FLAIR 或 SWI 等关键序列）。

Exclusion criteria：

1. Dementia caused by large vessel disease (e.g., severe stenosis or occlusion of internal carotid artery or middle cerebral artery), intracranial space occupying lesions, traumatic brain injury, Parkinson's disease, Alzheimer's disease, or other definite etiologies. 2. History of clinically diagnosed mental disorders (e.g., major depressive disorder, schizophrenia, bipolar disorder), or currently taking antipsychotic medications. 3. Severe aphasia, deafness, or visual impairment that precludes completion of neuropsychological assessment. 4. Concomitant severe hepatic or renal dysfunction (e.g., end stage renal disease), malignant tumors, or other major systemic diseases that affect life expectancy. 5. Contraindications to MRI (e.g., cardiac pacemaker, metallic implants, etc.). 6. Unavailability of serum homocysteine (Hcy) test results within 3 months before enrollment, or incomplete brain MRI data (missing key sequences such as FLAIR or SWI).
