研究目的：

主要研究目的 （1）在前瞻性、非干预性观察性研究框架下，评估基于术前磁共振成像（MRI）的三维弯曲路径肿瘤下缘至肛缘距离（DTAV）测量方法在真实临床场景中的可行性、一致性及重复性，并与结肠镜测距结果进行对照分析，验证该方法作为影像学测量工具的稳定性和可靠性。 次要研究目的 （1） 比较不同测量方式（如三维弯曲测距与传统二维 MRI测量）在 DTAV 评估中的差异，分析其在不同肿瘤分期、直肠解剖特征条件下的表现特点； （2）通过分层分析，探讨肿瘤 T 分期、肠腔弯曲度及狭窄程度等因素对测距结果差异的影响，识别可能导致测量偏差的关键因素； （3）评估该三维弯曲 DTAV 测量流程在不同研究中心中的一致性，为后续影像学测量标准化及多中心推广应用提供依据。  

Objectives of Study：

1.Prospective primary objective (1)Within the framework of a prospective, non-interventional observational study, to evaluate the feasibility, consistency, and reproducibility of a preoperative magnetic resonance imaging (MRI)-based three-dimensional curved-path measurement of the distance from the tumor lower margin to the anal verge (DTAV) in real-world clinical settings, and to validate its stability and reliability as an imaging measurement tool through comparative analysis with colonoscopic measurements. 2.Secondary objectives (1)To compare differences between measurement methods (e.g., three-dimensional curved-path vs. conventional two-dimensional MRI measurements) in DTAV assessment, and analyze their performance characteristics under varying tumor stages and rectal anatomical features; (2)To identify key factors causing measurement bias through stratified analysis, exploring the impact of tumor T staging, bowel lumen curvature, and degree of stenosis on measurement discrepancies; (3)To assess inter-center consistency of the three-dimensional curved DTAV measurement protocol, providing evidence for standardization of imaging measurements and multicenter clinical application.

Inclusion criteria

排除标准：

1.既往接受过可能明显改变直肠或肛管解剖结构的手术（如低位直肠切除、腹会阴联合切除等），影响肿瘤—肛缘距离准确评估者；
2.术前磁共振成像（MRI）资料不完整或影像质量不佳，无法满足影像学分析及测量要求者；
3.存在影响影像判读或测量的严重直肠或盆腔合并疾病（如严重炎症、畸形等）；
4.无法配合完成研究相关信息收集，或研究者认为不适合纳入本研究的其他情况；

Exclusion criteria：

1.Prior surgical history affecting anatomy:Patients who have undergone previous surgeries that may significantly alter the anatomical structure of the rectum or anal canal (e.g., low anterior resection, abdominoperineal resection), thereby affecting the accurate assessment of the tumor-anal verge distance.
2.Incomplete or poor-quality imaging: Patients with incomplete preoperative magnetic resonance imaging (MRI) data or poor image quality that fails to meet the requirements for imaging analysis and measurement. Confounding comorbidities:;
3.Presence of severe rectal or pelvic comorbidities (e.g., severe inflammation, malformations) that interfere with image interpretation or measurement.
4.Inability to comply or other disqualifiers: Patients unable to cooperate with the collection of study-related information, or other conditions deemed by the investigator as unsuitable for inclusion in this study.
