研究目的：

检验在农村地区的社区医疗环境下，采用血压波动特征指标预警的高血压控制策略相比常规血压控制策略是否可以提高高血压患者的降压达标率。  

Objectives of Study：

To examine whether the hypertension control strategy using blood pressure fluctuation characteristic indicators for early warning can improve the blood pressure control rate of hypertensive patients compared with the conventional blood pressure control strategy in the community medical environment of rural areas.

纳入标准：

Inclusion criteria

排除标准：

1. 年龄≥80 周岁； 2. 继发性高血压； 3. 肿瘤等严重疾病患者； 4. 妊娠或计划妊娠者； 5. 难治性高血压； 6. 慢性肾病患者； 7. 预期寿命<6 个月且行动不便无法到乡镇卫生院就诊患者； 8. 认知功能障碍，无法完成研究者； 9. 经专家/医生评估不适合参与研究的其他患者。

Exclusion criteria：

1. Aged >= 80 years; 2. Secondary hypertension; 3. Patients with severe diseases such as tumors; 4. Pregnant women or those planning to become pregnant; 5. Refractory hypertension; 6. Patients with chronic kidney disease; 7. Patients with a life expectancy of < 6 months and who are unable to visit the township health center due to limited mobility; 8. Patients with cognitive impairment who are unable to complete the research; 9. Other patients who are evaluated by experts/doctors as unfit to participate in the study.

基础治疗依据《中国高血压防治指南（2024 年修订版）》执行，推荐诊室降压目标为 <140/90 mmHg。研究期间，结合患者血压水平、心血管风险分层、合并疾病及既往治疗情况，给予规范化药物治疗、生活方式干预及健康教育。在基础干预的基础上，进一步实施以居家血压自测、数据自动上传、TTR 动态计算及分级预警反馈为核心的研究干预。

Basic treatment should be carried out in accordance with the Guidelines for the Prevention and Treatment of Hypertension in China (2024 Revised Edition). The recommended office blood pressure lowering target is <140/90 mmHg. During the study period, standardized drug treatment, lifestyle interventions, and health education were provided based on the patients' blood pressure levels, cardiovascular risk stratification, comorbidities, and previous treatment history. On the basis of basic interventions, research interventions centered on home blood pressure self - monitoring, automatic data upload, dynamic calculation of time in therapeutic range (TTR), and graded early - warning feedback were further implemented.

本研究采用群随机方法，限制区组随机化（CCR），限制在管患者数和空间距离。由统计师使用 SAS 软件生成群随机表并导入基于网络的交互式网络应答系统（Interactive Web Response System，IWRS）进行随机干预分配。

A cluster-randomized design was adopted in this study. Constrained cluster randomization (CCR) was used to allocate clusters while balancing the number of managed patients and geographic proximity between study arms. The randomization schedule was generated by an independent statistician using SAS software and uploaded into an Interactive Web Response System (IWRS), which was used to implement treatment allocation according to the predefined randomization sequence.
