研究目的：

探讨围术期右美托咪定联合帕瑞昔布钠对老年患者骨科手术术后认知功能和炎性反应的影响及对术后镇痛效果和不良反应的影响。  

Objectives of Study：

To investigate the effect of perioperative dexmedetomidine combined with parecoxib sodiumon on cognitive function and inflammatory response in elderly patients after orthopedic surgery and its effect on postoperative analgesia and adverse reactions.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

Inclusion criteria

排除标准：

（1）有非甾体类药物过敏史者
（2）有心脑血管疾病者或有脑部疾病病史如脑梗，中风和冠心病史等
（3）有慢性疼痛或精神、神经系统疾病长期使用镇痛药或精神系统药物（包括阿片类、非甾体抗炎药、镇静药、抗抑郁药）、精神系统药物滥用者
（4）术前肝、肾功能（谷丙转氨酶、谷草转氨酶、尿素氮、肌酐）超过正常值2倍者
（5）简易智能精神状态检查量表（MMSE）术前检查<24分
（6）存在神经和心理疾病的病史包括AD、帕金森、精神病等
（7）活动性消化道溃疡者
（8）15日内服用过单胺氧化酶抑制药物或抗抑郁药物
（9）严重的听力或者视力损害
（10）术后转入重症加强护理病房（intensive care unit, ICU）患者
（11）孕妇或产妇
（12）焦虑或抑郁患者
（13）不同意参加本研究

Exclusion criteria：

(1) Those with a history of allergic reactions to non-steroidal drugs;
(2) Those with cerebrovascular disease or have a history of brain diseases such as cerebral infarction, stroke and coronary heart disease history, etc.;
(3) long-term use of analgesics or psychotropic drugs (including opioids, non-steroidal anti-inflammatory drugs, sedatives, antidepressants) and psychotropic substance abusers in patients with chronic pain or mental and neurological diseases;
(4) Preoperative liver and kidney function (alanine aminotransferase, aspartate aminotransferase, urea nitrogen, creatinine) exceeds the normal value by 2 times;
(5) Simplified mental state checklist (MMSE) preoperative examination <24 points;
(6) History of neurological and psychological diseases including AD, Parkinson's, psychosis, etc.;
(7) active gastrointestinal ulcers;
(8) Take monoamine oxidase inhibitor or antidepressant within 15 days;
(9) severe hearing or visual impairment;
(10) Postoperative transfer to patients with intensive care unit (ICU);
(11) pregnant women or mothers;
(12) Anxiety or depression patients;
(13) Disagree to participate in this study.
