研究目的:

脑梗死后远隔损害会影响患者预后,进一步了解脑梗死后远隔损害的机制有助于找到早期、无创的影像生物标志物,从而可以制定更精准的干预措施,我们的研究旨在利用多模态磁共振探索大脑中动脉狭窄-闭塞患者远隔损害发生、发展的机制。  

Objectives of Study:

Remote damage after cerebral infarction can affect patient prognosis. A better understanding of the mechanisms underlying remote damage after cerebral infarction may help identify early, non-invasive imaging biomarkers, thereby enabling more precise intervention strategies. Our study aims to investigate the mechanisms underlying the occurrence and progression of remote damage in patients with severe stenosis or occlusion of the middle cerebral artery using multimodal magnetic resonance imaging.

纳入标准:

Inclusion criteria

排除标准:

病例组排除标准 1. 非粥样硬化性颅内动脉狭窄、闭塞:由栓塞、肿瘤、外伤、血管炎、先天畸形、感染、放射损伤引起的大脑中动脉狭窄、闭塞; 2. 双侧大脑中动脉存在重度狭窄或闭塞; 3. 丘脑有既往病灶; 4. 有行磁共振检查的禁忌症; 5. 预期寿命≤1年; 6. 既往诊断阿尔茨海默病、血管性痴呆、路易痴呆、额颞叶痴呆等神经认知障碍。 对照组排除标准 1. 有行磁共振检查的禁忌症。 2. 预期寿命≤1 年; 3. 既往诊断阿尔茨海默病、血管性痴呆、路易痴呆、额颞叶痴呆等神经认知障碍。

Exclusion criteria:

Exclusion criteria for the case group: 1. Non-atherosclerotic intracranial artery stenosis or occlusion: stenosis or occlusion of the middle cerebral artery caused by embolism, tumor, trauma, vasculitis, congenital malformation, infection, or radiation injury. 2. Bilateral severe stenosis or occlusion of the middle cerebral artery. 3. Pre-existing lesions in the thalamus. 4. Contraindications to magnetic resonance imaging. 5. Life expectancy <= 1 year. 6. Prior diagnosis of neurocognitive disorders such as Alzheimer's disease, vascular dementia, dementia with Lewy bodies, or frontotemporal dementia. Exclusion criteria for the control group: 1. Contraindications to MRI examination; 2. Life expectancy <= 1 year; 3. Prior diagnosis of neurocognitive disorders, including Alzheimer's disease, vascular dementia, dementia with Lewy bodies, frontotemporal dementia, or other related conditions.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

本研究问卷调查采用“纸质问卷+电子化录入”模式,自评问卷由经过培训的临床医师指导患者填写,神经功能评分及认知评分由经过培训的临床医师评估,影像学数据由专业放射科医师按实验计划参数采集,并以 DICOM 格式导出。实验室检查数据从医院 HIS 系统导出,并由专人双人独立录入。所有源文件(签署的知情同意书、纸质版调查问卷)、影像学DICOM文件、统计分析程序、代码均按规定存档,保存至研究结束后至少15 年。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

A paper‑based questionnaire with electronic entry was used; self‑reports were completed with clinician guidance, and neurological/cognitive assessments were performed by trained clinicians. Imaging data were acquired by radiologists per protocol and exported as DICOM. Lab data from HIS were double‑entered independently by two dedicated staff. All source documents, imaging files, analysis scripts, and code were archived and stored for >= 15 years post‑study.
