研究目的：

（1）评估超声输尿管跨越征动态变化（6小时内阳性 vs 24小时仍阴性 vs 从阴转阳）预测输尿管结石自行排出的价值； （2）确定跨越征预测自排的最佳时间截断值； （3）建立包含跨越征动态变化、结石大小、结石位置、肾积水程度的多因素预测模型； （4）比较跨越征动态变化与静态指标（单次跨越征结果）的预测效能  

Objectives of Study：

(1) To evaluate the value of dynamic changes of the ultrasonic ureteral crossing sign (positive within 6 hours, persistent negative at 24 hours, or negative-to-positive conversion) in predicting spontaneous passage of ureteral stones; (2) To determine the optimal time cutoff value of the crossing sign for predicting spontaneous passage using ROC curve analysis; (3) To establish a multivariate predictive model incorporating dynamic changes of the crossing sign, stone size, stone location, and degree of hydronephrosis; (4) To compare the predictive efficacy of dynamic changes of the crossing sign with static indicators (single assessment of the crossing sign)

纳入标准：

Inclusion criteria

排除标准：

1.合并输尿管狭窄、输尿管肿瘤、移植肾、输尿管畸形者； 2.双侧输尿管结石； 3.妊娠期女性； 4.合并急性尿路感染、发热（体温≥38.5℃）或脓毒症者； 5.就诊时已接受急诊手术干预者（未经保守治疗直接手术）； 6.随访数据不完整，无法确定最终结局者。

Exclusion criteria：

1. Patients with ureteral stricture, ureteral tumor, transplanted kidney, or ureteral malformation; 2. Bilateral ureteral stones; 3. Pregnant women; 4. Patients with acute urinary tract infection, fever (body temperature ≥38.5°C), or sepsis; 5. Patients who received emergency surgical intervention at the time of presentation (direct surgery without conservative treatment); 6. Incomplete follow-up data precluding determination of final outcome.

本研究为回顾性队列研究，从医院电子病历系统采集临床常规诊疗数据。数据采集使用标准化Case Report Form（CRF），包括人口学资料、临床特征、超声影像资料、CT影像资料及随访结局。数据录入采用双人双录入并进行一致性核对。数据存储于加密保护的专用计算机，仅研究团队成员可访问。所有数据不包含患者个人隐私信息（手机号、身份证号等），以匿名化编码进行管理。数据备份采用定期加密备份方式，防止数据丢失。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

This retrospective cohort study will collect data from the hospital's electronic medical record system using a standardized Case Report Form (CRF), including demographic data, clinical characteristics, ultrasound imaging data, CT imaging data, and follow-up outcomes. Double data entry with consistency verification will be performed. Data will be stored on an encrypted, password-protected computer accessible only to research team members. All data are anonymized without personal identifiable information (e.g., mobile phone numbers, ID numbers) and managed with coded identifiers. Regular encrypted backups will be performed to prevent data loss.
