研究目的：

估侧卧位与仰卧位对肥胖患者（BMI ≥ 30 kg/m²）麻醉苏醒期低氧血症发生率的影响，为术后呼吸管理提供循证依据。  

Objectives of Study：

To evaluate the effects of lateral position versus supine position on the incidence of hypoxemia during anesthesia emergence in obese patients (BMI ≥ 30 kg/m²), and to provide evidence-based guidance for postoperative respiratory management.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

Inclusion criteria

排除标准：

1. 术前低氧血症（吸空气SpO2 < 92%）； 2. 脊柱畸形或不稳、近期肋骨骨折等侧卧位禁忌； 3. 严重心脑血管疾病、颅内高压； 4. 术后需保留气管导管或转入ICU； 5. 妊娠或哺乳期。

Exclusion criteria：

1. Preoperative hypoxemia (SpO2 < 92% on room air); 2. Spinal deformity or instability, recent rib fracture, or other contraindications to lateral positioning; 3. Severe cardiovascular or cerebrovascular disease, intracranial hypertension; 4. Requirement for retained tracheal tube or transfer to ICU postoperatively; 5. Pregnancy or lactation.

Randomization is stratified by study center and performed using block randomization with a block size of 4, assigning patients in a 1:1 ratio to either the lateral position group or the supine position group.

Due to the nature of the positioning intervention, blinding of the operators was not feasible; however, outcome assessors (the nurses recording SpO₂ and events) were blinded to group allocation.
