研究目的:

本研究以系统性红斑狼疮(SLE)情绪障碍的发病机制为切入点,旨在明确5-HTTLPR基因多态性通过杏仁核结构和功能改变影响SLE情绪障碍的作用机制,并为SLE情绪障碍的早期识别与精准干预提供理论依据。具体目的包括: (1)明确SLE患者5-HTTLPR基因多态性与情绪障碍(焦虑、抑郁)及免疫炎症紊乱的相关性,从遗传-免疫交互角度阐明SLE情绪障碍的易感机制; (2)利用多模态磁共振成像技术,系统评估伴与不伴情绪障碍的SLE患者杏仁核亚区体积、微结构完整性及功能连接的改变特征,明确杏仁核亚区损伤在SLE情绪障碍中的神经病理基础; (3)通过构建中介效应模型,验证5-HTTLPR基因多态性是否通过杏仁核亚区结构及功能损害的介导途径导致SLE情绪障碍,并量化该中介效应,从而整合遗传、影像与免疫多维度证据,揭示SLE情绪障碍的潜在因果通路。最终,本研究期望发现可用于早期识别SLE情绪障碍的影像-遗传联合标志物,并为开发靶向神经-免疫交互通路的诊疗策略提供新思路。  

Objectives of Study:

This study aims to elucidate how 5-HTTLPR gene polymorphism affects mood disorders in systemic lupus erythematosus (SLE) through amygdala structural and functional alterations, providing a theoretical basis for early identification and targeted intervention. Specifically: (1) To determine the association between 5-HTTLPR polymorphism and mood disorders (anxiety/depression) as well as immune-inflammatory dysregulation in SLE patients; (2) To characterize amygdala subregional volumetric, microstructural, and functional connectivity changes in SLE patients with versus without mood disorders using multimodal MRI; (3) To verify and quantify the mediating role of amygdala subregional damage in the pathway from 5-HTTLPR polymorphism to SLE mood disorders through mediation analysis, thereby integrating genetic, neuroimaging, and immunological evidence to reveal the underlying neuro-immune mechanisms.Ultimately, this study seeks to identify potential imaging-genetic biomarkers for early recognition of SLE mood disorders and to inform novel therapeutic strategies targeting neuro-immune pathways.

Inclusion criteria

排除标准:

SLE 组: 1.合并其他自身免疫性疾病; 2.有高血压、糖尿病、脑卒中、肾功不全病史等可导致脑萎缩等严重临床情况者; 3.有严重颅脑外伤史、手术史; 4.有严重行为紊乱和意识障碍等重度活动性精神病表现者; 5.有药物滥用史、酗酒史及吸毒史者; 6.有精神活性药物使用史;有幽闭恐惧症、金属植入物等磁共振检查禁忌者; 7.妊娠期或哺乳期妇女; 健康对照组: 1.既往或现有明确的躯体疾病史、神经精神疾史(精神病症状由一位经验丰富的精神科医师使用 DSM-IV 的结构化临床访谈非病人版本进行筛选)、免疫系统疾病史及家族史; 2.有药物滥用史、酗酒史及吸毒史者; 3.有头部外伤史、手术史; 4.妊娠期或哺乳期妇女; 5.有幽闭恐惧症、金属植入物等磁共振检查禁忌者。

Exclusion criteria:

SLE Group: 1. People with other autoimmune diseases; 2. People with serious conditions that could cause brain atrophy, like a history of high blood pressure, diabetes, stroke, or kidney dysfunction; 3. People with a history of serious head trauma or surgery; 4. People showing severe active psychiatric symptoms like serious behavioral disorders or consciousness disturbances; 5. People with a history of drug abuse, heavy drinking, or substance use; 6. People who use psychoactive drugs; people who have claustrophobia, metal implants, or other MRI contraindications; 7. Pregnant or breastfeeding women. Healthy Control Group: 1. People with a clear past or current history of physical illness, neurological or psychiatric disorders (psychiatric symptoms screened by an experienced psychiatrist using the structured clinical interview for DSM-IV, non-patient edition), immune system diseases, or family history; 2. People with a history of drug abuse, heavy drinking, or substance use; 3. People with a history of head trauma or surgery; 4. Pregnant or breastfeeding women; 5. People who have claustrophobia, metal implants, or other MRI contraindications.
