研究目的：

1. 基于证据总结与Meta分析结果，初步构建《EICU成人患者中心静脉导管堵塞预防方案》初稿，并通过专家会议法形成《EICU成人患者中心静脉导管堵塞预防方案》； 2. 初步验证《EICU成人患者中心静脉导管堵塞预防方案》在降低EICU成人患者中心静脉导管堵塞发生率、延长导管通畅时间方面的临床效果。

Objectives of Study：

1. To initially construct the first draft of the Prevention Protocol for Central Venous Catheter (CVC) Occlusion in Adult Patients in the Emergency Intensive Care Unit (EICU) based on evidence summaries and Meta-analysis results, and to finalize the protocol through expert panel meetings; 2. To preliminarily validate the clinical effectiveness of the Prevention Protocol for CVC Occlusion in Adult Patients in the EICU in reducing the incidence of CVC occlusion and prolonging catheter patency time among adult patients in the EICU.

1. 以血液透析为目的置入的CVC； 2. 置管侧肢体同时存在其他静脉通路，如PICC； 3. 研究期间发生全身重度感染者； 4. 研究中途死亡的患者。

Exclusion criteria：

1. CVCs inserted for the purpose of hemodialysis; 2. Presence of other venous access, such as a PICC, in the same limb as the catheter insertion site; 3. Patients who develop severe systemic infection during the study period; 4. Patients who die during the course of the study.

干预措施：	1. 在2025年11月1日~10日对EICU护理人员进行为期10天的集中培训、考核与监控。 (1) 由护士长组织科室人员进行线下培训，并且录制了相关培训视频供医护人员线上自行学习，培训内容包括导致CVC堵塞的危险因素、冲封管时机以及各类型堵塞的预防措施，重点培训护士的冲封管操作手法。 (2) 培训结束后，进行相关理论以及操作考试，理论考核采用自制问卷《EICU成人患者中心静脉导管堵塞预防知识问卷》，问卷包括10道题，总分为100分，得分>=90分视为合格，问卷见附录5；操作考核由每小组的责任小组组长负责监督与考核。 (3) 考试合格后，由责任小组组长、责任区护士以及研究生在临床进行为期1周的监控，确保临床护士按照实施方案以及标准的操作手法对CVC进行维护，若出现冲封管不到位，需自行学习线上相关培训视频。做到当前轮转到EICU的小组中的15名护士都达到正确的操作手法时才进行收集数据。	干预措施代码：
Intervention：	1. From November 1 to November 10, 2025, a 10-day intensive training, assessment, and supervision program was conducted for EICU nursing staff, as follows: (1) Training: The head nurse organized in-person (offline) training sessions for department staff, and recorded relevant training videos for self-paced online learning by medical and nursing personnel. The training content included risk factors for CVC occlusion, timing of flushing and locking, and preventive measures for various types of occlusion, with a particular emphasis on nurses' flushing and locking techniques. (2) Assessment: Upon completion of the training, both theoretical and practical examinations were administered. The theoretical assessment consisted of a self-developed questionnaire titled "Knowledge Questionnaire on Prevention of Central Venous Catheter Occlusion in Adult EICU Patients" (see Appendix 5), which comprised 10 items with a total score of 100 points; a score of >= 90 points was considered passing. The practical assessment was supervised and evaluated by the team leader of each nursing group. (3) Supervision: After passing the examinations, the team leaders, designated zone nurses, and graduate students conducted one week of clinical supervision to ensure that all nursing staff maintained the CVC according to the implementation protocol and standardized operating techniques. If any nurse was found to have inadequate flushing or locking practices, they were required to independently review the relevant online training videos. Data collection was not initiated until all 15 nurses currently rotating through the EICU had demonstrated correct operating techniques.	Intervention code：

Intervention：	In the control group, CVC insertion and securement were performed by the EICU attending physicians in accordance with the standard operating procedures for central venous catheterization. Post-insertion nursing care was carried out by the primary nurses responsible for each patient, following the routine nursing protocol of the department. This included daily assessment of catheter function, monitoring for signs of occlusion, checking the security of catheter fixation, evaluating the insertion site for abnormalities such as redness, swelling, or exudate, and performing flushing and locking once daily with sequential flushing and locking of both lumens. All related measures were implemented with reference to the "Clinical Practice Guidelines for Common Complications of Intravenous Catheters" published by the Intravenous Infusion Therapy Professional Committee of the Chinese Nursing Association in 2022	Intervention code：
